OBJECTIVES: Emerging data suggest that not all grade 3 (G3) pancreatic neuroendocrine neoplasms (panNENs) behave the same; tumor differentiation may predict outcome. METHODS: Patients with G3 panNENs treated at our institution between 1999 and 2014 were identified. Demographics, response to therapy, and overall survival were determined. RESULTS: Forty-five patients were identified, 16 with G3 well differentiated pancreatic neuroendocrine tumors (WD-panNETs) and 29 with poorly differentiated neuroendocrine carcinomas (PDNEC). Median overall survival in G3 WD-panNET patients was 52.2 months (95% confidence interval, 19.3-86.9 months) compared with 10.1 months (95% confidence interval, 6.9-12.4 months) in PDNEC patients (P = 0.0009). Response rate to platinum agents was 10% in G3 WD-panNETs and 37% in PDNEC. Response rate to alkylating agents was 50% in G3 WD-panNETs and 50% in PDNEC. CONCLUSIONS: Both G3 WD-panNETs and PDNEC responded to platinum and alkylating agents. Overall survival was significantly greater in G3 WD-panNETs compared with PDNEC. These findings challenge current classification and suggest that G3 panNENs should be classified by morphology.
Among athletes strenuous exercise, dehydration and gastric emptying (GE) delay are the main causes of gastrointestinal (GI) complaints, whereas gut ischemia is the main cause of their nausea, vomiting, abdominal pain and (blood) diarrhea. Additionally any factor that limits sweat evaporation, such as a hot and humid environment and/or body dehydration, has profound effects on muscle glycogen depletion and risk for heat illness. A serious underperfusion of the gut often leads to mucosal damage and enhanced permeability so as to hide blood loss, microbiota invasion (or endotoxemia) and food-born allergen absorption (with anaphylaxis). The goal of exercise rehydration is to intake more fluid orally than what is being lost in sweat. Sports drinks provide the addition of sodium and carbohydrates to assist with intestinal absorption of water and muscle-glycogen replenishment, respectively. However GE is proportionally slowed by carbohydrate-rich (hyperosmolar) solutions. On the other hand, in order to prevent hyponatremia, avoiding overhydration is recommended. Caregiver's responsibility would be to inform athletes about potential dangers of drinking too much water and also advise them to refrain from using hypertonic fluid replacements.
OBJETIVOS: conhecer a prevalência do aleitamento materno e os fatores associados ao desmame das crianças menores de um ano, em Feira de Santana, no ano 2001. MÉTODOS: estudo transversal com aplicação de questionários às 2319 mães presentes nas 44 (71,1%) unidades de vacinação selecionadas, por estratificação simples. Foi calculada a razão de prevalência e considerado como significante p < 0,05 e intervalo de 95% de confiança. RESULTADOS: a prevalência do aleitamento foi 69,2% nos menores de um ano. A ocorrência da amamentação exclusiva foi maior nas crianças que não usavam chupeta (49,4%) e que mamaram no primeiro dia de vida (40,4%). As variáveis estatisticamente significantes relacionadas com as maiores chances de amamentar foram: amamentação no primeiro dia (p <0,001), mães que não trabalhavam fora do lar (p <0,001), menor renda familiar (p <0,001), multiparidade (p = 0,03) e a não utilização de chupetas (p <0,001). CONCLUSÕES: as atividades assistenciais quanto à prevenção do desmame devem estar voltadas sobretudo para as mães que trabalham fora do lar, aquelas de maior renda familiar e primíparas. Quanto às crianças, para as que não foram aleitadas no peito no primeiro dia de vida e que usam chupetas.
AbstractThe hypothesis that participation in leisure activities is a protective factor for women's mental health was evaluated in a community-based cross-sectional study of a poor neighborhood in the city of Salvador, Brazil. Female adult workers (n = 552) were interviewed from 470 randomly selected families. Participation in leisure activities is defined as any reported engagement in leisure activities during days off in the month preceding the interview. Scores from a validated mental morbidity instrument were used to assess symptoms of anxiety and depression. A negative association between participation in leisure activities and several anxiety/depression symptoms was found among women reporting no job satisfaction and low family income (Prevalence Ratio = 0.27; 95% Confidence Interval from 0.09 to 0.88). Participation in leisure activities may help maintain mental health under adverse life conditions.KEYWORDS: Leisuremental healthwomenprotective factors
Amazon social movements arose, like many others globally, in conflicts with political and economic elites over land use and resource extraction. Amazon social movements have moved beyond protest to protagonize large-scale forest protection. The article examines the history of the Transamazon highway colonists’ movement and its articulation of an alternative vision of regional sustainable development, leading to successful advocacy for the creation of a 5.6 million-hectare reserve mosaic in the Xingu river basin. The mosaic connected two blocks of indigenous territories ultimately forming a 30 million-hectare protected forest corridor, halting frontier expansion. While much recent conservation literature critiques international environmentalist agendas in tropical forest conservation, the Transamazon movement’s alliance with environmentalists was mutually beneficial. Amazon social movements’ substantial role in a global increase in protected tropical forest areas since the 1970s merits more attention from both international conservation organizations and scientists.
Abstract In forest fragments, rare habitats contribute to heterogeneity and may provide unique resources for frugivorous species like peccaries with spatially and temporally complex patterns of range use. This study examined seasonal habitat use by two sympatric peccary species ( Tayassu pecari and Tayassu tajacu ) in an Atlantic forest fragment on the plateau region of São Paulo state, Brazil. Previous studies showed that range use by T. pecari was highly nonrandom, and that both species persisted at population densities typical of larger forest fragments. To explain this, we quantified the use and availability of habitats and riparian zones with compositional analysis. Use by T. pecari was nonrandom with respect to availability. Habitat preferences were different during dry and wet seasons and corresponded closely with seasonal movements, core range and fruit use. Although aquatic habitats made up a small proportion of the herd home range, they were most preferred by T. pecari during both seasons. Headwater palmito Euterpe edulis swamps were most favored in the dry season, whereas swamps and marshes near larger streams were preferred in the wet season. Tayassu pecari preferred riparian zones <50 m from streams over drier zones. These habitats were important sources of fruits, travel routes and corridors between forest patches in the agricultural matrix. The least‐preferred habitat of T. pecari during both seasons was disturbed forest edge dominated by bamboo. Habitat and riparian zone use were herd‐specific for T. tajacu and related to habitat quality and composition where stable home ranges had been established. The persistence of viable peccary populations after 75 years of fragmentation‐associated pressures is related to preservation of rare habitats and overall habitat diversity. Thus, T. pecari is an indicator of high habitat diversity in forest fragments and will function as an umbrella species when targeted for conservation.
The objective of this study was to estimate the prevalence of malocclusion and associated variables such as deleterious habits (DH) and oronasopharyngeal alterations (OA), mouth breathing, atypical phonation, and atypical swallowing in three-year-old children in Vitória, Espírito Santo State, Brazil. The sample included 291 children of both sexes enrolled in a Children's Educational Center and selected through probability sampling by conglomerates. Logistic regression indicated a high relative risk (RR) in children with altered overjet, open bite, and cross-bite to present mouth breathing (RR = 1.89; CI: 1.56-2.03), (RR = 2.46; CI: 2.00-3.02), (RR = 1.45; CI: 1.23-1.72); atypical swallowing (RR = 2.57; CI: 1.87-3.52), (RR = 3.49; CI: 2.53-4.81), (RR = 1.86; CI: 1.46-2.39); and atypical phonation (RR = 2.25; CI: 1.66-3.05), (RR = 3.18; CI: 2.38-4.25), (RR = 1.71; CI: 1.32-2.22), respectively. An association was shown between finger or pacifier sucking and altered overjet (p < 0.001), and between pacifier sucking and open bite (p < 0.001). Such results indicate that the prevalence of malocclusions is associated with DH and OA.
Para melhorar a alimentação do pacu (Piaracatus mesopotamicus), é necessário conhecer o nível de aproveitamento dos ingredientes utilizados em suas dietas. Neste trabalho, inicialmente, foram analisados quatro métodos de coleta de fezes, em função da precisão e da versatilidade (dissecação, extrusão, Guelph e Guelph modificado), e o tempo entre as coletas de fezes nos sistemas de Guelph, por intermédio do coeficiente de digestibilidade da proteína bruta de uma dieta-referência para o pacu. Não houve diferença entre os métodos de coleta de fezes; já os intervalos de tempo das coletas apresentaram diferenças, podendo interferir nos resultados de digestibilidade da proteína. Em uma segunda etapa, para determinação dos coeficientes de digestibilidade da proteína e da energia de cinco concentrados protéicos de origem animal, cinco de origem vegetal e quatro energéticos, foram utilizadas rações elaboradas com 69,5% de uma dieta-referência, 30% do ingrediente teste e 0,5% de Cr2O3, utilizando o método de extrusão manual para a coleta de fezes. Os valores de digestibilidade da proteína da maioria dos alimentos apresentaram-se altos e não variaram estatisticamente, de 93,89 a 75,73% para o farelo de trigo e a farinha de penas, respectivamente. Entretanto, a farinha de sangue e a levedura apresentaram valores de digestibilidade da fração protéica significativamente inferiores (57,72 e 68,86%, respectivamente). Os coeficientes de digestibilidade da energia dos alimentos variaram consideravelmente, apresentando altos valores (acima de 90%) para o sorgo, o farelo de arroz e as sojas crua e tostada, e baixos valores (menores que 70%) para as farinhas de vísceras e de sangue, os farelos de soja e de algodão e a levedura. Quanto aos métodos de coleta de fezes, conclui-se que qualquer um pode ser adotado para a determinação de digestibilidade, desde que sejam rigorosamente aplicados. De maneira geral, a maioria dos concentrados protéicos pode ser utilizada como ótimas fontes de proteína, com exceção da farinha de sangue e a levedura, enquanto os alimentos energéticos confirmaram-se como excelentes fontes de energia em dietas para o pacu.
Avaliou-se neste estudo a eficiência de sistemas compostos de duas espécies de macrófitas aquáticas flutuantes (Eichhornia crassipes e Pistia stratiotes) no tratamento de efluentes gerados por um viveiro de manutenção de reprodutores de camarões-canela (Macrobrachium amazonicum). Os sistemas de tratamento foram constituídos de 12 unidades experimentais com as macrófitas aquáticas e três sem plantas (controle). Foram analisadas amostras da água de abastecimento do viveiro de criação de camarões-d'água-doce, do efluente gerado e dos efluentes tratados. A remoção de nutrientes não diferiu significativamente entre as duas espécies vegetais. As maiores remoções foram obtidas para fósforo total (41,9% pelo controle; 71,6% por E. crassipes; 69,9% por P. stratiotes; 72,5% por E. crassipes + P. stratiotes e 72,1% por P. stratiotes + E. crassipes) e para turbidez (30,6% pelo controle; 80,2% por E. crassipes; 75,2% por P. stratiotes; 79,8% por E. crassipes + P. stratiotes e 81,5% por P. stratiotes + E. crassipes). As macrófitas aquáticas foram eficientes na remoção de nitrogênio e fósforo dos efluentes de criação de M. amazonicum.
In this paper the dynamical interactions of a double pendulum arm and an electromechanical shaker is investigated. The double pendulum is a three degree of freedom system coupled to an RLC circuit based nonlinear shaker through a magnetic field, and the capacitor voltage is a nonlinear function of the instantaneous electric charge. Numerical simulations show the existence of chaotic behavior for some regions in the parameter space and this behaviour is characterized by power spectral density and Lyapunov exponents. The bifurcation diagram is constructed to explore the qualitative behaviour of the system. This kind of electromechanical system is frequently found in robotic systems, and in order to suppress the chaotic motion, the State-Dependent Riccati Equation (SDRE) control and the Nonlinear Saturation control (NSC) techniques are analyzed. The robustness of these two controllers is tested by a sensitivity analysis to parametric uncertainties.
OBJECTIVE: To estimate the prevalence of obstructive sleep apnea syndrome (OSAS) in children and adolescents with sickle cell anemia (SCA); to investigate the possible correlation between mean annual hemoglobin level and total sleep time with SpO2 < 90%, as well as between mean annual hemoglobin level and total sleep time with SpO2 < 80%; and to investigate the possible correlation between apnea-hypopnea index (AHI) and painful crisis. METHODS: The study involved 85 patients with SCA. The patients completed a questionnaire, were submitted to polysomnography and underwent clinical evaluation (by a pediatrician and an otolaryngologist). An AHI > 1 was considered indicative of a diagnosis of OSAS. RESULTS: The prevalence of OSAS was 10.6%. We found a negative correlation between mean annual hemoglobin level and total sleep time with SpO2 < 90% (r = micro0.343; p = 0.002), as well as between mean annual hemoglobin level and total sleep time with SpO2 < 80% (r = micro0.270; p = 0.016). There was no association between AHI and painful crisis. CONCLUSIONS: The prevalence of OSAS in this population was high (10.6%). Therefore, it is important to identify signs of OSAS as soon as possible and to determine the mean annual hemoglobin level because of the inverse correlation between that level and the total sleep time with SpO2 < 90% or < 80%.
OBJECTIVE: To describe the characteristics from people with head injury at Salvador city, as well, to point out the most frequent age groups, their disabilities and lethality rates. METHOD: Assessment and notification of 555 medical files from victims with head injury assisted at the General Hospital of Bahia during 2001. RESULTS: 82.9% from all victims were male and 17.1% female; the most frequent age group was 21 to 30 years; the most relevant cause of head injury was traffic accident (40.7%), followed by physical violence with or without weapons (25.4%) and falls (24%). CONCLUSION: The most evident profile group involved in accidents with head trauma implications was young male that usually had traffic injuries, with expressive rates of disabilities and lethality.
Filleting yield of Nile tilapia Oreochromis niloticus (L.) is low (30%) and generates large amount of wastes that may turn into environmental and economic problem. However, these wastes can be used for the extraction of minced fish (MF) which can be used in the preparation of sausages. The objective of this study was to assess the quality of sausages prepared with 0, 20, 40, 60, 80 and 100% of MF from Nile tilapia filleting waste during storage at 0±0.3ºC. Alterations in the instrumental color (L*, a* and b*), lipid oxidation (TBARS), total volatile nitrogenous bases (TVB-N), pH, microbiological condition (pathogenic bacteria and aerobic psychrotrophic bacteria), and sensory attributes (color, odor, flavor, texture and overall acceptability) were evaluated for up to 40 days. The addition of MF to sausages increased TBARS values and decreases TVB-N, L*, a* and b* values. Acceptability of color attribute decreased with increasing MF; best flavor, texture and overall acceptability scores were registered for sausages containing 40 and 60% MF; best odor was registered for 100% MF. Pathogenic microorganisms were not detected, but decrease in pH and proliferation of aerobic psychrotrophic bacteria which, however, did not compromise sensory evaluation of sausages were registered throughout storage. Sausages prepared with MF from tilapia filleting waste have a shelf-life of 40 days when stored at 0±0.3ºC, and the maximum recommended MF inclusion to maintain good sensory quality is 60%.
O presente trabalho foi desenvolvido com o objetivo de determinar os coeficientes de digestibilidade aparente da proteína e da energia dos principais alimentos utilizados na formulação de dietas para alevinos de pintado. Foram utilizados 600 alevinos com peso médio inicial de 9,80 ± 1,48 g e comprimento total médio de 13,00 ± 1,00 cm. Na coleta de fezes, foi utilizado o sistema de Guelph modificado. As 12 dietas-teste foram constituídas por 69,50% de uma dieta de referência, 0,50% de óxido de cromo (marcador inerte) e 30% do ingrediente estudado. Após receberem as dietas teste durante três dias, os peixes foram transferidos para os aquários de coleta (incubadoras de fibra de vidro de 80 litros de capacidade), onde as fezes foram coletadas em intervalos de meia hora. Com base nos coeficientes de digestibilidade da fração protéica, os alimentos que apresentaram maior aproveitamento para esse nutriente foram: farinha de peixe (84,14%), farelo de soja (67,10%), milho (64,18%) e farinha de vísceras de aves (61,25%). Foram observados valores razoáveis somente para a digestibilidade do conteúdo energético em metade dos ingredientes estudados; para as farinhas de peixe, milho, soja integral tostada e os farelos de soja, de trigo e de arroz, os coeficientes médios foram: 72,80; 57,39; 64,95; 61,66; 53,20 e 51,84%, respectivamente. A farinha de peixe foi o melhor ingrediente para o pintado (45,38% PD e 2790,42 kcal ED/kg), seguido do farelo de soja (30,86% PD e 2708,45 kcal ED/kg), da soja integral tostada (18,34% PD e 3121,06 kcal ED/kg), do milho (5,86% PD e 2691,53 kcal ED/kg) e do farelo de trigo (8,08% PD e 2265,13 kcal ED/kg).
The aim of the present study was to evaluate the influence of probiotics prepared from Bacillus subtilis (strain C-3102-Calsporin®; Calpis, Tokyo, Japan) as a dietary supplement on several reproductive aspects of female ‘matrinxã’(Brycon amazonicus), including hematological parameters, immunological characteristics and total lipid concentrations in muscle and liver tissue. The broodstock were kept in two ponds of 600 m2, at a density of 50 fish per pond, from March to November 2008. The fish were divided into two treatment groups: untreated control fish (T1), and fish fed with 10 g of probiotics per kg of ration (T2). Fish were fed a diet containing 32% crude protein (CP), 7% crude fat (CF), and 6.5% ether extract (EE) twice daily at a rate of 1% of the total biomass per day during the coldest months (May through August) and 3% during the warm season (March, April and September through November). During the reproductive period, 20 individuals from each treatment group were selected for the experiments; two doses of crude carp pituitary extract were given to females in 10-h intervals, and one dose was given to the males at the same time the females were given the second dose. A total of 20 fish were used for the hematological analyses. After anesthetization, blood samples were withdrawn by caudal puncture to determine the total number of cells, differential and total leukocyte counts, thrombocyte count, hematocrit, hemoglobin rate and plasma cortisol and glucose levels. The hematimetric index mean corpuscular volume (VCM) and mean corpuscular hemoglobin concentration (MHCM) were calculated. Ten other ‘matrinxãs’ from the same groups were used to assess macrophage phagocytic activity and lipid levels. The results showed that females fed a probiotics-supplemented diet exhibited an increase in numbers of oocytes and, consequently, had higher rates of fertilization and hatching of larvae. Probiotic-treated fish also exhibited a significant increase in the phagocytic activity of macrophages, indicating an improvement in the immune system of breeders. Hematologic parameters were different in comparison to the time-zero group, and plasma cortisol and glucose levels were higher in the females. The total lipids levels in muscle and liver tissue were lower in B. amazonicus that received feed supplemented with probiotics.
Tambaqui Colossoma macropomum, Cuvier, 1818 (Osteichthyes: Characidae) were used in this study. The blood of the animals was obtained by punction on the tail vessel with a heparinized syringe (5000 IU) and with one containing EDTA 10%. After collection, the blood was immediately processed in order to measure hematocrit percentage and hemoglobin concentration. It was then left in room temperature where it was kept for a ten-hour period, after which hematocrit and hemoglobin concentrations were measured again. Results indicated that, in teleosts, anticoagulant selection can interfere significantly with the results of the studied hematological parameters. However, blood storage at room temperature did not have a significant effect (α =0.05) on hematocrit percentage, hemoglobin rate or mean corpuscular hemoglobin concentration (MCHC).
We analyzed, by env and gag heteroduplex mobility assay, 149 human immunodeficiency virus (HIV-1) positive samples collected in Ceará during the year 2000. The prevalence of subtype B was 81.2% and the prevalence of subtype F and B/F recombinants were both 2.7%. Eight (5.4%) and 12 (8%) out of 149 samples showed indeterminate results in the env and gag analysis respectively. By FokI restriction fragment length polymorphism, 34% of the subtype B samples were identified as the typical Brazilian subtype B. In the present study, we identified HIV-1 subtype F and B/F in Ceará for the first time. Our results contribute to the understanding of HIV in Brazil, and may prove useful for the development of vaccine candidates.
Ethanol is one of the chemicals related to the development of oral malignant neoplasms. Micronuclei are chromatin fragments which, after aberrant mitoses, do not become included in the main nucleus. They have been used as indicators of genotoxic damage in cells exposed to carcinogens. The aim of this study was to assess the frequency of micronuclei in exfoliated cells from the tongue and buccal mucosa of alcoholic individuals. Samples were taken from the tongue and buccal mucosa of 40 alcoholic individuals who did not smoke, and from 20 alcohol and tobacco abstainers. Cells were stained with the Feulgen reaction and counterstained with Fast Green. A significant increase in the frequency of micronuclei in tongue cells was found in the group of subjects exposed to alcohol, when compared to the control group (p < 0.01). The frequency of micronuclei in buccal mucosa cells was higher in the group of alcoholic individuals, when compared to the control group, although there was no significant difference (p > 0.05). Our results indicate that excessive alcohol consumption may induce effective alterations on oral mucosa cells, even without exposure to tobacco. These alterations are more expressive in the tongue, which is a site more exposed to the action of carcinogens, when compared to the buccal mucosa.
OBJECTIVE: To investigate the symptoms most frequently found in children with a polysomnographic diagnosis of obstructive sleep apnea-hypopnea syndrome (OSAHS). METHODS: We evaluated 38 children consecutively referred to the sleep laboratory with suspicion of OSAHS between June of 2003 and December of 2004. The patients were submitted to a pre-sleep questionnaire and to polysomnography. RESULTS: The mean age was 7.8 +/- 4 years (range, 2-15 years), and 50% of the children were male. Children without apnea accounted for 7.9% of the sample. The obstructive sleep apnea observed in the remainder was mild in 42.1%, moderate in 28.9% and severe in 22.1%. Severe cases of apnea were most common among children under the age of six (pre-school age). In children with OSAHS, the most common symptoms were snoring and nasal obstruction, which were observed in 74.3 and 72.7% of the children, respectively. Excessive sleepiness and bruxism were seen in 29.4 and 34.3%, respectively, and reflux disease was seen in only 3.1%. Restless legs and difficulty in falling asleep were identified in 65 and 33%, respectively. All of the children diagnosed with severe OSAHS also presented snoring and bruxism. CONCLUSIONS: Snoring and nasal obstruction were the most common symptoms found in our sample of children and adolescents with OSAHS. In addition, OSAHS severity was associated with being in the lower age bracket.
Abstract The expression of the EBV‐associated membrane antigen complex was studied in three lines of cultured Burkitt lymphoma (BL) cells after exposure to various metabolic inhibitors and X‐irradiation. Direct membrane immunofluorescence (MIF) with a BL reference conjugate (F‐Mutua) revealed a high percentage of positive cells in two of the cell lines (Maku and Onesmas), provided they were cultured under conditions previously defined as optimal for the expression of the membrane antigen. The third cell line (Silfere) contained less than 5% MIF‐positive cells regardless of the culture conditions. Mitomycin C, actinomycin D, methotrexate and X‐irradiation increased the frequency of positive cells in Maku and Onesmas cultures kept under conditions favoring a low membrane antigen expression. Puromycin decreased the frequency of MIF‐positive cells in highly positive cultures from 50% to less than 10%. The increase or decrease in MIF became apparent 3 days after application of the inhibitor. Cytosine arabinoside and low temperature (31°C and 28°C) had no effect on MIF in cultures with either high or low membrane antigen expression, although cell growth was suppressed. In the Silfere cell line, none of the inhibitors studied had any effect on the expression of the membrane antigens.
