Continuing population and consumption growth will mean that the global demand for food will increase for at least another 40 years. Growing competition for land, water, and energy, in addition to the overexploitation of fisheries, will affect our ability to produce food, as will the urgent requirement to reduce the impact of the food system on the environment. The effects of climate change are a further threat. But the world can produce more food and can ensure that it is used more efficiently and equitably. A multifaceted and linked global strategy is needed to ensure sustainable and equitable food security, different components of which are explored here.
§ 34. 1. The permeability and capillarity constants of soil have been defined. 2. The movements of air and water through three types of soil have been measured and shewn to conform to equations connecting the rate of motion with the above constants. 3. It is suggested that the measurement of S , P α , P ω and K is of more importance than, and should replace, the determination of the sizes of the soil particles as in the usual “mechanical analysis” of soils. Eucalypts are the world’s most widely planted hardwood trees. Their outstanding diversity, adaptability and growth have made them a global renewable resource of fibre and energy. We sequenced and assembled >94% of the 640-megabase genome of Eucalyptus grandis. Of 36,376 predicted protein-coding genes, 34% occur in tandem duplications, the largest proportion thus far in plant genomes. Eucalyptus also shows the highest diversity of genes for specialized metabolites such as terpenes that act as chemical defence and provide unique pharmaceutical oils. Genome sequencing of the E. grandis sister species E. globulus and a set of inbred E. grandis tree genomes reveals dynamic genome evolution and hotspots of inbreeding depression. The E. grandis genome is the first reference for the eudicot order Myrtales and is placed here sister to the eurosids. This resource expands our understanding of the unique biology of large woody perennials and provides a powerful tool to accelerate comparative biology, breeding and biotechnology. The availability of comprehensive genomic data will be of use in work on accelerating breeding cycles for productivity and wood quality and developing eucalypt strains suited to a variety of habitats.
The unremitting trends of increasing population, urbanization, diminishing water supply, and continuing climate change have contributed to declining stocks of arable land per person. As land resources for agriculture decrease, policy makers are faced with the challenge of sustainability and feeding the rapidly growing world population which is projected to reach approximately 9.7 billion in 2050. Solutions for improving future food production are exemplified by urban vertical farming which involves much greater use of technology and automation for land-use optimization. The vertical farm strategy aims to significantly increase productivity and reduce the environmental footprint within a framework of urban, indoor, climate-controlled high-rise buildings. It is claimed that such facilities offer many potential advantages as a clean and green source of food, along with biosecurity, freedom from pests, droughts, and reduced use of transportation and fossil fuels. In this article, the issues involved are evaluated together with potential advantages and disadvantages. Possible implications are identified for consideration by policy makers and to facilitate further economic analysis.
Epidemiology as an empirical science has developed sophisticated methods to measure the causes and patterns of disease in populations. Nevertheless, for many diseases in many countries only partial data are available. When the partial data are insufficient, but data collection is not an option, it is possible to supplement the data by exploiting the causal relations between the various variables that describe a disease process. We present a simple generic disease model with incidence, one prevalent state, and case fatality and remission. We derive a set of equations that describes this disease process and allows calculation of the complete epidemiology of a disease given a minimum of three input variables. We give the example of asthma with age-specific prevalence, remission, and mortality as inputs. Outputs are incidence and case fatality, among others. The set of equations is embedded in a software package called 'DisMod II', which is made available to the public domain by the World Health Organization.
Importance: Emerging evidence suggests that risk of bacterial sexually transmitted infections (STIs) increases among gay and bisexual men following initiation of HIV preexposure prophylaxis (PrEP). Objective: To describe STI incidence and behavioral risk factors among a cohort of predominantly gay and bisexual men who use PrEP, and to explore changes in STI incidence following PrEP commencement. Design, Setting, and Participants: The Pre-exposure Prophylaxis Expanded (PrEPX) Study, a multisite, open-label intervention study, was nested within the Australian Collaboration for Coordinated Enhanced Sentinel Surveillance (ACCESS) clinic network. A total of 4275 participants were enrolled (July 26, 2016-April 1, 2018) in Victoria, Australia. Of these, 2981 enrolled at 5 ACCESS clinics (3 primary care, 1 sexual health, and 1 community-based HIV rapid testing service), had at least 1 follow-up visit, and were monitored until April 30, 2018. Exposures: Upon enrollment, participants received daily oral tenofovir disoproxil fumurate and emtricitabine for HIV PrEP, quarterly HIV and STI testing, and clinical monitoring. Main Outcomes and Measures: The primary outcome was incidence of chlamydia, gonorrhea, or syphilis. Incidence rates and hazard ratios describing behavioral risk factors of STI diagnosis were calculated. Incidence rate ratios (IRRs), adjusted for change in testing frequency, described changes in STI incidence from 1-year preenrollment to study follow-up among participants with preenrollment testing data (n = 1378). Results: Among the 2981 individuals (median age, 34 years [interquartile range, 28-42]), 98.5% identified as gay or bisexual males, 29% used PrEP prior to enrollment, 89 (3%) withdrew and were censored at date of withdrawal, leaving 2892 (97.0%) enrolled at final follow-up. During a mean follow-up of 1.1 years (3185.0 person-years), 2928 STIs were diagnosed among 1427 (48%) participants (1434 chlamydia, 1242 gonorrhea, 252 syphilis). STI incidence was 91.9 per 100 person-years, with 736 participants (25%) accounting for 2237 (76%) of all STIs. Among 2058 participants with complete data for multivariable analysis, younger age, greater partner number, and group sex were associated with greater STI risk, but condom use was not. Among 1378 participants with preenrollment testing data, STI incidence increased from 69.5 per 100 person-years prior to enrollment to 98.4 per 100 person-years during follow-up (IRR, 1.41 [95% CI, 1.29-1.56]). After adjusting for testing frequency, the increase in incidence from 1 year preenrollment to follow-up was significant for any STI (adjusted IRR, 1.12 [95% CI, 1.02-1.23]) and for chlamydia (adjusted IRR, 1.17 [95% CI, 1.04-1.33]). Conclusions and Relevance: Among gay and bisexual men using PrEP, STIs were highly concentrated among a subset, and receipt of PrEP after study enrollment was associated with an increased incidence of STIs compared with preenrollment. These findings highlight the importance of frequent STI testing among gay and bisexual men using PrEP.
Artificial intelligence and automation are topics dominating global discussions on the future of professional employment, societal change, and economic performance. In this paper, we describe fundamental concepts underlying AI and Big Data and their significance to public health. We highlight issues involved and describe the potential impacts and challenges to medical professionals and diagnosticians. The possible benefits of advanced data analytics and machine learning are described in the context of recently reported research. Problems are identified and discussed with respect to ethical issues and the future roles of professionals and specialists in the age of artificial intelligence.
<ns4:p><ns4:bold>Background: </ns4:bold>The <ns4:italic>w</ns4:italic>Mel strain of<ns4:italic> Wolbachia</ns4:italic> has been successfully introduced into <ns4:italic>Aedes aegypti</ns4:italic> mosquitoes and subsequently shown in laboratory studies to reduce transmission of a range of viruses including dengue, Zika, chikungunya, yellow fever, and Mayaro viruses that cause human disease. Here we report the entomological and epidemiological outcomes of staged deployment of <ns4:italic>Wolbachia</ns4:italic> across nearly all significant dengue transmission risk areas in Australia.</ns4:p><ns4:p> <ns4:bold>Methods: </ns4:bold>The <ns4:italic>w</ns4:italic>Mel strain of <ns4:italic>Wolbachia</ns4:italic> was backcrossed into the local <ns4:italic>Aedes aegypti</ns4:italic> genotype (Cairns and Townsville backgrounds) and mosquitoes were released in the field by staff or via community assisted methods. Mosquito monitoring was undertaken and mosquitoes were screened for the presence of <ns4:italic>Wolbachia</ns4:italic>. Dengue case notifications were used to track dengue incidence in each location before and after releases.</ns4:p><ns4:p> <ns4:bold>Results: </ns4:bold>Empirical analyses of the <ns4:italic>Wolbachia</ns4:italic> mosquito releases, including data on the density, frequency and duration of <ns4:italic>Wolbachia</ns4:italic> mosquito releases, indicate that <ns4:italic>Wolbachia</ns4:italic> can be readily established in local mosquito populations, using a variety of deployment options and over short release durations (mean release period 11 weeks, range 2-22 weeks). Importantly, <ns4:italic>Wolbachia</ns4:italic> frequencies have remained stable in mosquito populations since releases for up to 8 years. Analysis of dengue case notifications data demonstrates near-elimination of local dengue transmission for the past five years in locations where <ns4:italic>Wolbachia</ns4:italic> has been established. The regression model estimate of <ns4:italic>Wolbachia </ns4:italic>intervention effect from interrupted time series analyses of case notifications data prior to and after releases, indicated a 96% reduction in dengue incidence in <ns4:italic>Wolbachia </ns4:italic>treated populations (95% confidence interval: 84 – 99%).</ns4:p><ns4:p> <ns4:bold>Conclusion: </ns4:bold>Deployment of the <ns4:italic>w</ns4:italic>Mel strain of <ns4:italic>Wolbachia</ns4:italic> into local <ns4:italic>Ae. aegypti</ns4:italic> populations across the Australian regional cities of Cairns and most smaller regional communities with a past history of dengue has resulted in the reduction of local dengue transmission across all deployment areas.</ns4:p>
Hospitalization for Ambulatory Care Sensitive Conditions (ACSH) is an accepted indicator of access to health care and avoidable morbidity. Accessible care of reasonable quality should reduce ACSH. Little research has examined the indicator's external validity. We calculated standardized ACSH rates for 32 areas of Victoria, Australia (population 4.4 million). A representative survey measured access, disease prevalence, propensity to seek care, disease burden, social determinants of health services use, and behavioral risk factors. Regression analyses compared self-rated access with ACSH rates. Independent of prevalence, propensity to seek care, disease burden, and physician supply, better access was associated with lower ACSH rates. Results provide support for the ACSH indicator. When rural residence was considered, the covariate measuring access was not significant. However, rural residence also may contribute importantly to access. Results suggest both the complexity of the meaning of access and the desirability of further research to validate the ACSH indicator.

Each year, approximately 60,000 children of refugee background are resettled in Western countries. This paper reviews the effects of the refugee experience on cognitive functioning. The distinctive influences for these children include exposure to traumatic events and the need to acquire a new language, factors that need to be considered to avoid overdiagnosis of learning disorders and inappropriate educational placements. Prearrival trauma, psychological sequelae of traumatic events, developmental impact of trauma, and the quality of family functioning have been found to influence cognitive functioning, learning, and academic performance. In addition, the refugee child may be semiproficient in several languages, but proficient in none, whilst also trying to learn a new language. The influence that the child’s limited English proficiency, literacy, and school experience may have on academic and test performance is demonstrated by drawing on the research on refugees’ English language acquisition, as well as the more extensive literature on bilingual English language learners. Implications for interventions are drawn at the level of government policy, schools, and the individual. The paper concludes with the observation that there is a major need for longitudinal research on refugee children’s learning and academic performance and on interventions that will close the academic gap, thereby enabling refugee children to reach their educational potential.
The relationship between administrative service performance and citizen satisfaction has been assumed, but not demonstrated, in the application of market models to public service delivery. Although the citizen satisfaction literature cautions that the link between objective and subjective measures of service quality is tenuous at best, public-sector professional organizations define a managerial focus on objective measures of service performance as accountability to citizens for outcomes. What if we're wrong?
Creating ‘liveable’ communities that are healthy and sustainable is an aspiration of policymakers in Australia and overseas. Indicators are being used at the national, state and local level to compare the liveability of cities and regions. Yet, there are challenges in the adoption of such indicators. Planning scholars see a challenge in creating indicators that measure something publicly valued, while public health researchers are concerned about scant systemic research on relationships between policies, the built environment, and health and well-being. This article provides an overview of liveability indicators used to date in Australia and internationally. It then outlines the results of consultations with Melbourne-based academics and decision-makers, on how to increase their utility and support the creation of healthy, liveable and sustainable cities.
BACKGROUND: Although allied health clinicians play a key role in the provision of healthcare, embedding a culture of research within public and private health systems remains a challenge. In this rapid review we critically evaluate frameworks for embedding research into routine allied health practice, as the basis for high quality, safe, efficient and consumer-focused care. METHODS: A rapid review (PROSPERO: CRD42017075699) was conducted to evaluate frameworks designed to create and embed research in the health sector. Included were full-text, English-language, peer-reviewed publications or Government reports of frameworks that could inform the implementation of an allied health research framework. Eight electronic databases and four government websites were searched, using search terms such as models, frameworks and research capacity-building. Two independent researchers conducted all review stages and used content and thematic analysis to interpret the results. RESULTS: Sixteen framework papers were finally included. Content analysis identified 44 system and regulatory level items that informed the research frameworks, 125 healthcare organisation items and 76 items relating to individual clinicians. Thematic analysis identified four major themes. Firstly, sustainable change requires allied health research policies, regulation, governance and organisational structures that support and value evidence-based practice. Secondly, research capability, receptivity, advocacy and literacy of healthcare leaders and managers are key to successful research implementation. Third, organisational factors that facilitate a research culture include dedicated staff research positions, time allocated to research, mentoring, professional education and research infrastructure. When healthcare agencies had strong partnerships with universities and co-located research leaders, research implementation was strengthened. Finally, individual attributes of clinicians, such as their research skills and capabilities, motivation, and participation in research teams, are essential to embedding research into practice. CONCLUSION: Theoretical frameworks were identified that informed processes to embed a culture of allied health research into healthcare services. Research-led and evidence-informed allied health practice enables optimisation of workforce capability and high-quality care.
Abstract In this study, first year commerce students in Australia were surveyed about their perceptions of their accounting studies and their perceptions of the attributes required of professional accountants. The paper specifically addresses the factors important in determining whether first year students intend to become accountants. The study uses a logistic regression model incorporating demographic and academic factors, as well as students' perceptions of the work of accountants, to predict intention to become an accountant. The results show that the perception of importance of generic skills, intrinsic interest in the discipline area, and course satisfaction were significant in determining intention to pursue a career as an accountant. As many students formed their judgments about the work of accountants from their accounting studies, the findings have implications for accounting educators in terms of the enthusiasm and motivation required in teaching accounting, as well as curriculum development that reflects the skill set required for an increasingly sophisticated business environment.
Rapid diagnosis and whole genome sequencing confirmed a case of monkeypox in an HIV-positive individual receiving antiretroviral therapy. The patient had a normal CD4+ T-cell count and suppressed HIV viral load and presented with a genital rash in Melbourne, Australia after return from Europe in May 2022. He subsequently developed systemic illness and disseminated rash and 11 days after symptom onset, he was hospitalised to manage painful bacterial cellulitis of the genital area.
BACKGROUND: A large outbreak of hepatitis A affected individuals in several Australian states in 2009, resulting in a 2-fold increase in cases reported to state health departments compared with 2008. Two peaks of infection occurred (April-May and September-November), with surveillance data suggesting locally acquired infections from a widely distributed food product. METHODS: Two case-control studies were completed. Intensive product trace-back and food sampling was undertaken. Genotyping was conducted on virus isolates from patient serum and food samples. Control measures included prophylaxis for close contacts, public health warnings, an order by the chief health officer under the Victorian Food Act 1984, and trade-level recalls on implicated batches of semidried tomatoes. RESULTS: A multijurisdictional case-control study in April-May found an association between illness and consumption of semidried tomatoes (odds ratio [OR], 3.0; 95% CI 1.4-6.7). A second case-control study conducted in Victoria in October-November also implicated semidried tomatoes as being associated with illness (OR, 10.3; 95% CI, 4.7-22.7). Hepatitis A RNA was detected in 22 samples of semidried tomatoes. Hepatitis A virus genotype IB was identified in 144 of 153 (94%) patients tested from 2009, and partial sequence analysis showed complete identity with an isolate found in a sample of semidried tomatoes. CONCLUSIONS: The results of both case-control studies and food testing implicated the novel vehicle of semidried tomatoes as the cause of this hepatitis A outbreak. The outbreak was extensive and sustained despite public health interventions, the design and implementation of which were complicated by limitations in food testing capability and complex supply chains.
In childhood, mental health problems primarily consist of behaviour and emotional problems. These affect one in every seven children (i.e. 200 000 in Australia). Left untreated, up to 50% of preschool problems continue through the childhood years. Because of their high prevalence, population-based approaches will be needed to reduce their associated burden. The aim of the present study was therefore to identify evidence-based preventive interventions for behavioural and emotional problems of children aged 0-8 years. Randomized controlled trials of preventive interventions for behavioural and emotional problems were located by searching standard clinical databases and systematic reviews. The authors determined which programmes were effective and ineffective, dividing the effective programmes into those with high or low risk of trial bias. Among effective programmes, the most promising for delivery in Australian contexts were identified, selected for their strength of evidence, sample comparability to Australia's population, and programme compatibility with Australia's service system. Around 50 preventive interventions have been evaluated in randomized controlled trials. Most targeted children's behavioural problems, and a few targeted emotional problems. Three US programmes have the best balance of evidence: in infancy, the individual Nurse Home Visitation Programme; at preschool age, the individual Family Check Up; at school age, the Good Behaviour Game class programme. Three parenting programmes in England and Australia are also worthy of highlight: the Incredible Years group format, Triple P individual format, and Parent Education Programme group format. Effective preventive interventions exist primarily for behaviour and, to a lesser extent, emotional problems, and could be disseminated from research to mainstream in Australia, ensuring fidelity to original programmes. Future research should develop programmes targeting emotional problems, and replicate effective programmes for behaviour problems in quality population translation trials. Randomized trial methods in staged roll-outs can determine population cost-benefits for children's mental health without delaying dissemination.
Decision support systems (DSS) have long been used in research, service provision and extension. Despite the diversity of technological applications in which past agricultural DSS canvass, there has been relatively little information on either the functional aspects of DSS designed for economic decisions in irrigated cropping, or the human and social factors influencing the adoption of knowledge from such DSS. The objectives of the study were to (1) review the functionality and target end-users of economic DSS for irrigated cropping systems, (2) document the extent to which these DSS account for and visualise uncertainty in DSS outputs, (3) examine tactical or strategic decisions able to be explored in DSS (with irrigation infrastructure being a key strategic decision), and (4) explore the human and social factors influencing adoption of DSS heuristics. This study showed that development of previous DSS has often occurred as a result of a technology push instead of end-user pull, which has meant that previous DSS have been generated in a top-down fashion rather than being demand-driven by end-user needs. We found that few DSS enable analysis of both tactical and strategic decisions, and that few DSS account for uncertainty in their outputs. We uncover a surprising lack of documented end-user feedback on economic DSS for irrigated cropping, such as end-user satisfaction with DSS functionality or future intentions to use the technology, as well as a lack of DSS application outside regions in which they were originally developed. Declining adoption of DSS does not necessarily imply declining adoption of DSS heuristics; in fact, declining DSS uptake may indicate that knowledge and heuristics extended by the DSS has been successful, obviating the need for use of the DSS per se. Future DSS could be improved through the use of demand-driven participatory approaches more aligned with user needs, with more training to build human capacity including understanding uncertainty and ability to contrast tactical and strategic decisions using multiple economic, environmental and social metrics.
Abstract Research in the field of public administration has changed and advanced significantly in recent years. These advancements concern both how we engage in research—such as the methods we apply, the interdisciplinary nature of the theories we use, and the research questions we ask. Increasingly, we are witnessing a shift in public‐sector values away from efficiency and effectiveness and toward a paradigm that highlights equity. In this article, we reflect on these changes from our position as editors‐in‐chief of some of the leading journals in the field. In addition to describing the progress of this discipline, we explore emerging windows of opportunity for new research. One such window is research on ways to incorporate interdisciplinary perspectives and methods. We also see the need for work in such areas as social equity, comparative administration, artificial intelligence, and climate change. Finally, we argue for a more proactive approach in disseminating research to those involved in the day‐to‐day decision‐making processes of public organizations.
