Inflammatory bowel disease patient‐reported quality assessment should drive service improvement: a national survey of <scp>UK IBD</scp> units and patients

BACKGROUND: Healthcare service provision in inflammatory bowel disease (IBD) is often designed to meet targets set by healthcare providers rather than those of patients. It is unclear whether this meets the needs of patients, as assessed by patients themselves. AIMS: To assess patients' experience of IBD and the healthcare they received, aiming to identify factors in IBD healthcare provision associated with perceived high-quality care. METHODS: Using the 2019 IBD standards as a framework, a national benchmarking tool for quality assessment in IBD was developed by IBD UK, comprising a patient survey and service self-assessment. RESULTS: 134 IBD services and 9757 patients responded. Perceived quality of care was lowest in young adults and increased with age, was higher in males and those >2 years since diagnosis. No hospital services met all the national IBD standards for recommended workforce numbers. Key metrics associated with patient-reported high- quality care were: identification as a tertiary centre, patient information availability, shared decision- making, rapid response to contact for advice, access to urgent review, joint medical/surgical clinics, and access to research (all p < 0.001). Higher numbers of IBD nurse specialists in a service was strongly associated with patients receiving regular reviews and having confidence in self-management and reporting high- quality care. CONCLUSIONS: This extensive patient and healthcare provider survey emphasises the importance of aspects of care less often measured by clinicians, such as communication, shared decision- making and provision of information. It demonstrates that IBD nurse specialists are crucial to meeting the needs of people living with IBD.

Consistent measures of oxidative balance predict survival but not reproduction in a long‐distance migrant

Physiological processes, including those that disrupt oxidative balance, have been proposed as key to understanding fundamental life-history trade-offs. Yet, examination of changes in oxidative balance within wild animals across time, space and major life-history challenges remains uncommon. For example, migration presents substantial physiological challenges for individuals, and data on migratory individuals would provide crucial context for exposing the importance of relationships between oxidative balance and fitness outcomes. Here we examined the consistency of commonly used measures of oxidative balance in longitudinally sampled free-living individuals of a long-lived, long-distance migrant, the Brent goose Branta bernicla hrota over periods of months to years. Although inter-individual and temporal variation in measures of oxidative balance were substantial, we found high consistency in measures of lipid peroxidation and circulating non-enzymatic antioxidants in longitudinally sampled individuals. This suggests the potential for the existence of individual oxidative phenotypes. Given intra-individual consistency, we then examined how these physiological measures relate to survival and reproductive success across all sampled individuals. Surprisingly, lower survival was predicted for individuals with lower levels of damage, with no measured physiological metric associated with reproductive success. Our results demonstrate that snapshot measurements of a consistent measure of oxidative balance can inform our understanding of differences in a key demographic trait. However, the positive relationship between oxidative damage and survival emphasises the need to investigate the relationships between the oxidative system and fitness outcomes in other species undergoing similar physiologically challenging life cycles. This would highlight the extent to which variation in such traits and resource allocation trade-offs is a result of adaptation to different life-history strategies.

A Prospective Cohort Study to Assess Neonatal Adaptation in Neonates Exposed to Psychotropic Medications in Utero

Aims We aimed to compare the NAS in neonatal exposed to antidepressants to unexposed neonates. Methods A prospective cohort study was carried out comprising of women in 3rd trimester of pregnancy, data were collected on women exposed and unexposed to antidepressants. Approval from the Rotunda Hospital REC was obtained. Hospital records were used to collect pre-, peri- and postnatal information which was relevant to the study Aim. Neonatal Abstinence Score was completed within 0–48 hours of the birth, Moderate-Severe abstinence was defined as eight points or higher (on a scale with maximum 40 points), mild abstinence as 4 points or higher. Paediatric records were reviewed where the baby required NICU admission. Women were recruited between 2019–2021 Results 221 women in total were recruited,138 pregnant women were on no psychotropic medication (Control group) and 83 pregnant women were on antidepressant medication (exposed group). In the exposed group, 46% (38/83) were on Sertraline,19% (16/83) fluoxetine, 17% (14/83) Escitalopram and 17% (14/83) on other SSRI/SNRI. Six infants (3%) expressed signs of severe abstinence and 38 (28%) had mild abstinence symptoms in exposed group whereas in control group 10 (7%) of infants were observed to have mild abstinence which was seen in infants with low birth weight, poor feeding and poor sleep after feed. Neonatal hypoglycaemia in infants prenatally exposed to antidepressant was seen in 10% compared to 1% of control group. Conclusion Severe abstinence in infants prenatally exposed to antidepressants was found to be (3%) and mild abstinence in 28% this is in keeping with international findings. Low one minute APGAR sores and greater rates of hypoglycaemia were also noted. While neonatal withdrawal with all antidepressants are usually mild and self-limiting it is important to make the obstretric and neonatal teams aware of the mothers medication and mothers should be advised that their baby may need a review by neonatology after delivery.

Singing Steadfast Love and Faithfulness: The Messianic Significance of Ps 89

Psalm 89 is a study in complexity. Debate ensues concerning its unity, structure, genre and historical setting. Its literary setting causes disagreement, which is not helped by the pivotal role it plays in understanding the shape of the Psalter. Most disorientating, however, is the contradictory emotions-exultant praise gives way to deep lament. The questions raised by these contradictory emotions remain unanswered. In this article, it is suggested that Ps 89 is best understood as having messianic significance. The case is made by noting that the psalm employs theological themes which culminate in the messiah, is heavily based on 2 Sam 7, fails to resolve, holds an important position in the Psalter's shape and clearly contributes to the development of messianic hope in its early interpretation. The messianic significance of Ps 89, I contend, is found in its singing the steadfast love and faithfulness of YHWH despite the bleak circumstances to which it alludes.

THE DISTINCTIVE SHAPE OF KINGSHIP IN ANCIENT ISRAEL: A CONSIDERATION OF KINGSHIP IN THE PENTATEUCH

This article explores the shape of kingship in ancient Israel with reference to the Pentateuch and particularly Deuteronomy 17:14–20. It demonstrates that Israel’s kingship is distinctive from that of the surrounding nations. The distinctive nature is linked, in the first place, to the creation of the nation and, secondly, to the stipulations for kings contained in Deuteronomy 17. It concludes that although there is some similarity between kingship in Israel and the surrounding nations, at root kingship in Israel is fundamentally distinctive. Whereas in the ancient Near East the king was god, in Israel God was king.

Impact of Physico-chemical Parameters on the Rotifer Assemblage in an Irrigation Canal at Phubala, Bishnupur District, Manipur, North East India

Biodiversity on the earth consists of a number of distinct biological species which together make a balanced environment. North East India is a place of exceptional biodiversity, harbouring a wide variety of biota. The geographic location of Manipur belongs to two of the world’s hotspot i.e. Eastern Himalayan and Indo-Burmese. As a result of this, the state is endowed with a wide variety of biodiversity. Nowadays, the global environmental crisis is affecting all dimensions of biota. Similarly, the rich biodiversity of the state is also under threat due to various anthropogenic activities. This environmental crisis may lead to multiple forms of destruction, resulting in measurable damage to the environment. This chapter reports the impact of physico-chemical parameters on the rotifer assemblage in an irrigation canal at Phubala, Bishnupur district, Manipur. Rotifers are an important group of zooplankton and act as good indicators of the changes in water quality, as they are strongly affected by environmental conditions and respond quickly to changes in water quality. The impact of physico-chemical parameters on the assemblage of rotifers was conducted in an irrigation canal at Phubala (latitude- 24.606330N and longitude 93.794410E), Bishnupur district of Manipur, North East India, for a period of one year. During the study period, 23 rotifers belonging to 7 families were recorded. The seven different families are: Brachionidae (10 genera), Euchlanidae (2 genera), Mytilinidae (2 genera), Epiphanidae (1 genus), Lecanidae (6genera), Filinidae (1 genus) and Testudinellidae (1 genus). Physico-chemical parameters have a significant influence on the occurrence of rotifers in the aquatic ecosystem. Therefore, the correlation matrix among physico-chemical parameters such as water temperature, pH, Dissolved Oxygen (DO), Free Carbon Dioxide (FCO2) and Total Alkalinity (TA) with the occurrence of rotifer is also studied.
