Human movement has been the subject of investigation since the fifth century when early scientists and researchers attempted to model the human musculoskeletal system. The anatomical complexities of the human body have made it a constant source of research to this day with many anatomical, physiological, mechanical, environmental, sociological and psychological studies undertaken to define its key elements. These studies have utilised modern day techniques to assess human movement in many illnesses. One such modern technique has been direct measurement by accelerometry, which was first suggested in the 1970s but has only been refined and perfected during the last 10-15 years. Direct measurement by accelerometry has seen the introduction of the successful implementation of low power, low cost electronic sensors that have been employed in clinical and home environments for the constant monitoring of patients (and their controls). The qualitative and quantitative data provided by these sensors make it possible for engineers, clinicians and physicians to work together to be able to help their patients in overcoming their physical disability. This paper presents the underlying biomechanical elements necessary to understand and study human movement. It also reflects on the sociological elements of human movement and why it is important in patient life and well being. Finally the concept of direct measurement by accelerometry is presented with past studies and modern techniques used for data analysis.
The efficiency of a standardised inhalation test procedure was studied by examining the reproducibility of responses to histamine and methacholine. In addition, the responses to the two agents were compared. Each set of duplicate tests was carried out on a separate day within one week, and all factors known or presumed to influence responses were carefully controlled. The results were expressed as the provocative concentration of the agent causing a 20% fall in forced expired volume in one second (PC20). Responses to histamine and methacholine were highly reproducible (coefficients of determination [r2] = 0.994 and 0.990 respectively). Responsiveness to histamine correlated closely with responsiveness to methacholine (r2 = 0.85). There was a small but significant cumulative dose effect with methacholine (P less than 0.01) but not with histamine. Side effects of throat irritation, flushing, and headache were more frequent with histamine than methacholine, and were dose-related. The high level of reproducibility indicates the efficiency of the test procedure. The similar severity of effects by agents with different mechanisms of action suggests that the primary cause of non-specific bronchial hyperreactivity lies at the level of bronchial smooth muscle.
Fibrodysplasia ossificans progressiva (FOP) is an autosomal dominant human disorder of bone formation that causes developmental skeletal defects and extensive debilitating bone formation within soft connective tissues (heterotopic ossification) during childhood. All patients with classic clinical features of FOP (great toe malformations and progressive heterotopic ossification) have previously been found to carry the same heterozygous mutation (c.617G>A; p.R206H) in the glycine and serine residue (GS) activation domain of activin A type I receptor/activin-like kinase 2 (ACVR1/ALK2), a bone morphogenetic protein (BMP) type I receptor. Among patients with FOP-like heterotopic ossification and/or toe malformations, we identified patients with clinical features unusual for FOP. These atypical FOP patients form two classes: FOP-plus (classic defining features of FOP plus one or more atypical features) and FOP variants (major variations in one or both of the two classic defining features of FOP). All patients examined have heterozygous ACVR1 missense mutations in conserved amino acids. While the recurrent c.617G>A; p.R206H mutation was found in all cases of classic FOP and most cases of FOP-plus, novel ACVR1 mutations occur in the FOP variants and two cases of FOP-plus. Protein structure homology modeling predicts that each of the amino acid substitutions activates the ACVR1 protein to enhance receptor signaling. We observed genotype-phenotype correlation between some ACVR1 mutations and the age of onset of heterotopic ossification or on embryonic skeletal development.
I wish to describe what I believe to be a novel approach to abdominal field block. The technique, as originally described, entails multiple injections and administration of potentially toxic doses of local anaesthetic agent [1]. This new approach involves identifying the neurovascular plane of the abdominal musculature and injecting a local anaesthetic agent therein. The only area of the abdominal wall where the internal oblique muscle can be localised directly is the ‘lumbar triangle of Petit’ where it forms the floor of this triangle. In most people, the lumbar triangle is situated just behind the highest point of the iliac crest. Local anaesthetic agent deposited in the area of the lumbar triangle will block the lower intercostal nerves, the iliohypogastric and the ilioinguinal nerves as they traverse between subcostal margin and the iliac crest. The iliac crest [2] has ventral and dorsal segments. The ventral segment has external and internal lips and a rough intermediate zone. The crest's summit is level with the L3−4 intervertebral space. The lower fibres of the external oblique and the latissimus dorsi muscles are attached to the external lip. A variable interval exists between the most posterior attachment of external oblique and the most anterior attachment of latissimus dorsi. Here, the crest forms the base of the ‘lumbar triangle of Petit’. The floor of the triangle is the internal oblique muscle, which is attached to the crest's intermediate area. The transversus abdominis muscle is attached to the anterior two-thirds of the crest's inner lip. The lumbar triangle is bounded anteriorly by the free posterior border of external oblique, posteriorly by the lower, lateral margin of latissimus dorsi and inferiorly by the iliac crest. The seventh to eleventh intercostals nerves, subcostal nerve, iliohypogastric and ilioinguinal nerves, all run a variable part of their courses between internal oblique and transversus abdominis muscles. With the patient in the supine position, a finger is walked posteriorly from the anterior superior iliac spine along the top of the iliac crest until it dips slightly inward. On further posterior movement, the finger-tip is felt to slip over the edge of a muscle. At this point, the finger is assumed to be abutting on the lateral border of latissimus dorsi where it is attached to the external lip of the iliac crest (Fig. 8). Without moving the hand, the skin is pierced anterior to the finger-tip with an 18G cutting needle at the level of the external lip. A 24G, blunt-tipped, 2-inch needle (1Plexufix®, Ref: 0489 1562, B. Braun.) is inserted perpendicular to the skin until it touches bone of the external lip (Fig. 9). Thereafter, the needle is slowly advanced over the intermediate zone of the iliac crest until a definite ‘pop’ or ‘sensation of giving way’ is felt. At this juncture, the needle has reached the plane between the internal oblique and the transversus abdominis muscles. After negative aspiration, 20 ml of a local anaesthetic agent is injected. Only one injection is required for a unilateral incision, e.g. in appendicectomy, while bilateral injections are administered for midline or transverse abdominal incisions. For a successful block, the injectate must disappear between the muscle layers without any apparent swelling of the abdominal wall. I have used this technique for more than 2 years and performed the abdominal field block via the lumbar triangle on more than 200 patients without any untoward sequelae. For bilateral injections, the maximum dose limit of the local anaesthetic agent is carefully observed. If the needle is advanced too far, a second ‘pop’ is felt. This indicates that the needle has passed through the transversus abdominis muscle attached to the internal lip of the iliac crest and must be withdrawn and re-inserted. Even after the second ‘pop’, the needle would have to pass through the transversalis fascia and parietal peritoneum before reaching the peritoneal cavity. The use of a fine-gauged, blunt-tipped needle helps to minimise the possibility of visceral damage if the needle is advanced too far inadvertently. The block may not be easy in obese patients because of difficulty in identifying landmarks. In these subjects, the point of needle insertion is chosen 2.5 cm behind the highest point of the iliac crest, a landmark easily palpable in most people. In elderly patients, the whole thickness of the iliac crest can be grasped between two fingers due to the loss of muscle mass and tone. It makes block easier to perform, bearing in mind that the needle tip must not be advanced beyond the inner lip of the crest. In small children, the lumbar triangle is felt like a tiny hole in the abdominal wall just behind the highest point of the iliac crest. I recommended a 24G 1-inch needle (1Plexufix®, Ref: 0489 152, B. Braun) for use in children. If extraordinary resistance is felt during injection, the needle may have lifted the periosteum in which case it should be withdrawn and re-sited. Finally, I would emphasise that this new approach of abdominal field block via the lumbar triangle should be avoided in patients with a lumbar hernia in which the hernial sac protrudes through the lumbar triangle.
Guidelines are presented for safe practice in the use of intravenous drug infusions for general anaesthesia. When maintenance of general anaesthesia is by intravenous infusion, this is referred to as total intravenous anaesthesia. Although total intravenous anaesthesia has advantages for some patients, the commonest technique used for maintenance of anaesthesia in the UK and Ireland remains the administration of an inhaled volatile anaesthetic. However, the use of an inhalational technique is sometimes not possible, and in some situations, inhalational anaesthesia is contraindicated. Therefore, all anaesthetists should be able to deliver total intravenous anaesthesia competently and safely. For the purposes of simplicity, these guidelines will use the term total intravenous anaesthesia but also encompass techniques involving a combination of intravenous infusion and inhalational anaesthesia. This document is intended as a guideline for safe practice when total intravenous anaesthesia is being used, and not as a review of the pros and cons of total intravenous anaesthesia vs. inhalational anaesthesia in situations where both techniques are possible.
Clinical subtyping of delirium according to motor-activity profile has considerable potential to account for the heterogeneity of this complex and multifactorial syndrome. Previous work has identified a range of clinically important differences between motor subtypes in relation to detection, causation, treatment experience and prognosis, but studies have been hampered by inconsistent methodology, especially in relation to definition of subtypes. This article considers research to date, including a number of recent studies that have attempted to address these issues and identify a means of achieving greater consistency in approaches to subtyping. Possibilities for future work are discussed and a research plan for the field is outlined.
Accurate monitoring of the mobility status of older adults, over the long-term, is important in rehabilitation medicine, as regular physical activity is central to maintaining both physical and mental health, as well as evaluating quality of life. This technical note describes an accelerometer-based mobility monitoring technique, which can distinguish between static and dynamic activities and can detect the basic postures of sitting, standing and lying. The technique allows thresholds for these postures to be set and two different posture threshold methods are described: mid-point and "best estimate". Preliminary results from using these methods are presented. This preliminary evaluation of the technique was carried out over the long-term (>29 h) in an uncontrolled environment and the method used to carry out the evaluation is described in detail. The two different posture thresholding methods were tested on long-term mobility data from one older adult subject. The subject did not have to follow a specific activity protocol during the recording period (4 days) and was shadowed by an observer in order to evaluate the accuracy of this technique. The monitoring hardware consisted of two accelerometer devices, one on the trunk and the other on the thigh and a pocket-sized ambulatory data-logger. Applying 'best estimate' thresholding, as opposed to mid-point thresholding, improved sitting detection accuracy by 18%, to 93% and lying detection accuracy by 5%, to 84%. Thus, based on these preliminary data, an accurate mobility monitoring system for older adults is described and it was observed that the actual posture threshold limits applied have a high impact on the mobility monitoring system's accuracy and are particularly important for accurately detecting postures when used over the long-term, in an uncontrolled environment.
The response to inhaled methacholine is increased in patients with chronic airflow obstruction, but it is not known whether this is due to true hyperresponsiveness or is a result of the airflow obstruction. In asthmatics the response to methacholine correlates with the bronchoconstriction produced by hyperventilation of cold dry air. We studied 27 patients with a history of smoking and chronic bronchitis with a range of severity of airflow obstruction. Bronchial responses to methacholine (expressed as the provocation concentration causing a fall in FEV1 of 20%-PC20) and isocapnic hyperventilation of cold dry air were measured. In 19 patients the PC20 was less than 8 mg/ml (that is, in the asthmatic range) but only three developed bronchoconstriction in response to hyperventilation. There was a linear correlation between the log PC20 and the FEV1 (r = 0.86, p less than 0.001). The results suggest that in patients with chronic airflow obstruction the response to methacholine is determined by the degree of airflow obstruction, and cannot be used in the diagnosis of asthma in the absence of additional information.
OBJECTIVES: Numerous long-term studies of depression in psychiatric settings have shown a poor clinical outcome but little emphasis has been placed on psychosocial or functional outcome in studies to date. This article reviews published data on long-term social functioning after depression and considers why psychosocial recovery appears delayed compared with clinical recovery. METHODS: Searches were carried out of the databases MEDLINE, PSYCHLIT and EMBASE for articles published from 1980 using keywords relating to social and functional outcomes of unipolar and bipolar depression. Review articles and relevant textbooks were also searched. RESULTS: The few outcome studies published have described long-term functional impairment in the majority of patients but have been limited by methodological shortcomings. Psychosocial impairment tends to persist even after clinical remission from depression. Residual symptomatology after remission from depression may lead to enduring psychosocial impairment, as may subtle neurocognitive deficits. Axis I and II comorbidities predict a poor psychosocial outcome, but episodes of depression do not appear to lead to personality 'scarring'. CONCLUSIONS: Future outcome studies need to focus on longitudinal social functioning. Full functional recovery after an episode of depression should be the goal of treatment as enduring residual symptoms lead to long-term psychosocial impairment.
Arterial occlusive disease remains a major health issue in the developed world and a rapidly growing problem in the developing world. Although a growing number of patients are now being effectively treated with minimally invasive techniques, there remains a tremendous pressure on the vascular community to develop a synthetic small-diameter vascular graft with improved long-term patency rates. The field of tissue engineering offers an exciting alternative in the search for living organ replacement structures. Several methodologies have emerged for constructing blood vessel replacements with biological functionality. Common strategies include cell-seeded biodegradable synthetic scaffolds, cell self-assembly, cell-seeded gels and xenogeneic acellular materials. A wide range of materials are being investigated as potential scaffolds for vascular tissue engineering applications. Some are commercialised and others are still in development. Recently, researchers have studied the role of fibrin gel as a three-dimensional scaffold in vascular tissue engineering. This overview describes the properties of fibrin gel in vascular tissue engineering and highlights some recent progress and difficulties encountered in the development of cell fibrin scaffold technology.
We discontinued anticonvulsant drugs in 92 patients who had been free of seizures during two years of treatment with a single drug. All the patients had epilepsy that had previously been untreated, and had been randomly assigned to receive carbamazepine, phenytoin, or sodium valproate. Thirty-one patients relapsed, and 61 remained free of seizures. The mean duration of the follow-up in the patients remaining free of seizures was 35 months (range, 6 to 62). There was no significant difference between the relapse rate among adults (35 percent) and that among children (31 percent). Our results suggest that the number of seizures a patient had before control was achieved, the number of drugs tried as single-drug therapy, and the type of treatment withdrawn all influenced the outcome. Among the various types of seizures, complex partial seizures with secondary generalization carried the worst prognosis. In comparison, the risk of relapse was 65 percent lower in patients with generalized seizures and 97 percent lower in patients with complex or simple partial seizures in the absence of secondary generalized attacks. Among the four electroencephalographic classes, class 4 (abnormal before treatment and unchanged before withdrawal) carried the worst prognosis. The risk of relapse was 94 to 99 percent lower in patients in the other three electroencephalographic classes. Among the three anticonvulsants, withdrawal of sodium valproate carried the worst prognosis. In comparison, the odds of relapsing were 28 percent lower after withdrawal of phenytoin and 85 percent lower after withdrawal of carbamazepine. We conclude that withdrawal of anticonvulsant medication should be considered in patients free of seizures for two years.
BACKGROUND: A complete pathological response occurs in 10-30 per cent of patients with locally advanced rectal cancer undergoing neoadjuvant chemoradiotherapy (CRT). The standard of care has been radical surgery with high morbidity risks and the challenges of stomata despite the favourable prognosis. This review assessed minimalist approaches (transanal excision or observation alone) to tumours with a response to CRT. METHODS: A systematic review was performed using PubMed and Embase databases. Keywords included: 'rectal', 'cancer', 'transanal', 'conservative', 'complete pathological response', 'radiotherapy' and 'neoadjuvant'. Original articles from all relevant listings were sourced. These were hand searched for further articles of relevance. Main outcome measures assessed were rates of local recurrence and overall survival, and equivalence to radical surgery. RESULTS: Purely conservative 'watch and wait' strategies after CRT are still controversial. Originally used for elderly patients or those who refused surgery, the data support transanal excision of rectal tumours showing a good response to CRT. A complete pathological response in the T stage (ypT0) indicates < 5 per cent risk of nodal metastases. CONCLUSION: Rectal tumours showing an excellent response to CRT may be suitable for local excision, with equivalent outcomes to radical surgery. This approach should be the subject of prospective clinical trials in specialist centres.
OBJECTIVE: To determine whether the use of ultrasound can reduce the incidence of incorrect diagnosis of the fetal head position at instrumental delivery and subsequent morbidity. DESIGN: Two-arm, parallel, randomised trial, conducted from June 2011 to December 2012. SETTING: Two maternity hospitals in the Republic of Ireland. SAMPLE: A cohort of 514 nulliparous women at term (≥37 weeks of gestation) with singleton cephalic pregnancies, aiming to deliver vaginally, were recruited prior to an induction of labour or in early labour. METHODS: If instrumental delivery was required, women who had provided written consent were randomised to receive clinical assessment (standard care) or ultrasound scan and clinical assessment (ultrasound). [Correction added on 17 April 2014, after first online publication: Sentence was amended.] MAIN OUTCOME MEASURE: Incorrect diagnosis of the fetal head position. RESULTS: The incidence of incorrect diagnosis was significantly lower in the ultrasound group than the standard care group (4/257, 1.6%, versus 52/257, 20.2%; odds ratio 0.06; 95% confidence interval 0.02-0.19; P < 0.001). The decision to delivery interval was similar in both groups (ultrasound mean 13.8 minutes, SD 8.7 minutes, versus standard care mean 14.6 minutes, SD 10.1 minutes, P = 0.35). The incidence of maternal and neonatal complications, failed instrumental delivery, and caesarean section was not significantly different between the two groups. CONCLUSIONS: An ultrasound assessment prior to instrumental delivery reduced the incidence of incorrect diagnosis of the fetal head position without delaying delivery, but did not prevent morbidity. A more integrated clinical skills-based approach is likely to be required to prevent adverse outcomes at instrumental delivery.
BACKGROUND: Predictive scores such as APACHE II and SAPS II have been used to assess patients in intensive care units, but only the modified early warning (MEW) score has been used to assess acutely ill general medical patients. DESIGN: Observational study of predictors of mortality. SETTING: Small Irish rural hospital. METHODS: From 17 February 2000 to 29 January 2004, 9,964 consecutive patients admitted as acute medical emergencies were divided into a derivation cohort of 6,736 patients and a validation cohort of 3,228 patients. RESULTS: In the derivation cohort, 316 patients (4.7%) died within 30 days of hospital admission. Under univariate analysis, age, vital signs and 18 categorical variables were associated with increased risk of death, and nine with reduced risk. Logistic regression identified 16 independent predictors of 30-day mortality, from which the Simple Clinical Score was derived, stratifying patients into five risk classes. In each class, mortality was not significantly different between the derivation and validation cohorts: 0-0.1% for very low risk, 1.5-1.6% for low risk, 3.8-3.9% for average risk, 9.0-10.3% for high risk, and 29.2-34.4% for very high risk. DISCUSSION: The Simple Clinical Score quickly and accurately identifies patients at both a low and high risk of death from the first to the 30th day after admission, enabling prompt triage and placement within a health-care facility.
A Spiral Computerized Tomography (CT) scan of the aorta were obtained from a single subject and three model variations were examined. Computational fluid dynamics modeling of all three models showed variations in the velocity contours along the aortic arch with differences in the boundary layer growth and recirculation regions. Further down-stream, all three models showed very similar velocity profiles during maximum velocity with differences occurring in the decelerating part of the pulse. Flow patterns obtained from transient 3-D computational fluid dynamics are influenced by different reconstruction methods and the pulsatility of the flow. Caution is required when analyzing models based on CT scans.
The aim of this investigation was to elucidate the reductions in muscle, skin and core temperature following exposure to -110°C whole body cryotherapy (WBC), and compare these to 8°C cold water immersion (CWI). Twenty active male subjects were randomly assigned to a 4-min exposure of WBC or CWI. A minimum of 7 days later subjects were exposed to the other treatment. Muscle temperature in the right vastus lateralis (n=10); thigh skin (average, maximum and minimum) and rectal temperature (n=10) were recorded before and 60 min after treatment. The greatest reduction (P<0.05) in muscle (mean ± SD; 1 cm: WBC, 1.6 ± 1.2°C; CWI, 2.0 ± 1.0°C; 2 cm: WBC, 1.2 ± 0.7°C; CWI, 1.7 ± 0.9°C; 3 cm: WBC, 1.6 ± 0.6°C; CWI, 1.7 ± 0.5°C) and rectal temperature (WBC, 0.3 ± 0.2°C; CWI, 0.4 ± 0.2°C) were observed 60 min after treatment. The largest reductions in average (WBC, 12.1 ± 1.0°C; CWI, 8.4 ± 0.7°C), minimum (WBC, 13.2 ± 1.4°C; CWI, 8.7 ± 0.7°C) and maximum (WBC, 8.8 ± 2.0°C; CWI, 7.2 ± 1.9°C) skin temperature occurred immediately after both CWI and WBC (P<0.05). Skin temperature was significantly lower (P<0.05) immediately after WBC compared to CWI. The present study demonstrates that a single WBC exposure decreases muscle and core temperature to a similar level of those experienced after CWI. Although both treatments significantly reduced skin temperature, WBC elicited a greater decrease compared to CWI. These data may provide information to clinicians and researchers attempting to optimise WBC and CWI protocols in a clinical or sporting setting.
BACKGROUND: In this article, we describe a case report of using real-time, high-resolution ultrasound guidance to facilitate blockade of the suprascapular nerve. We describe a case report and technique for using a portable ultrasound scanner (38 mm broadband (13-6 MHz) linear array transducer (SonoSite Micromaxx SonoSite, Inc. 21919 30th Drive SE Bothwell W. A..)) to guide suprascapular nerve block. METHODS: A 44-year old male patient presented with severe, painful osteoarthritis with adhesive capsulitis of his right shoulder. The ultrasound transducer in a transverse orientation was placed over the scapular spine. Moving the transducer cephalad the suprascapular fossa was identified. While imaging the supraspinatus muscle and the bony fossa underneath, the ultrasound transducer was moved laterally (maintaining a transverse transducer orientation) to locate the suprascapular notch. The suprascapular nerve was seen as a round hyperechoic structure at 4 cm depth beneath the transverse scapular ligament in the scapular notch. The nerve had an approximate diameter of 200 mm. Real-time imaging was used to direct injection in the scapular notch. Ultrasound scanning confirmed local anesthetic spread. RESULTS: The patient's pain intensity decreased. Shoulder movement and function improved. These improvements were maintained at 12 weeks. CONCLUSION: Ultrasound guidance does not expose patients and personnel to radiation. It is also less expensive than other imaging modalities. This technique has applications in both acute and chronic pain management.
BACKGROUND: We developed a technique for ultrasound-guided paravertebral block, which was subsequently applied in the clinical setting. METHODS: An initial cadaver study was used to develop a technique that was used in the clinical setting on patients undergoing breast cancer surgery. RESULTS: Paravertebral catheters were correctly placed in the cadaveric trial in 8 of 10 attempts. In the clinical study, all blocked patients (n = 9) had evidence of thoracic wall sensory block and analgesia postoperatively. CONCLUSIONS: Determined by anatomical dissection, we have described the ultrasound features of the thoracic paravertebral space and performed clinically successful ultrasound-guided paravertebral block.
AIM: To develop and validate an in-hospital mortality risk prediction tool for unselected acutely ill medical patients using routinely collected physiological and laboratory data. DESIGN: Analysis of all emergency medical patients admitted to St James's Hospital (SJH), Dublin, between 1 January 2002 and 31 December 2007. Validation using a dataset of acute medical admissions from Nenagh Hospital 2000-04. METHODS: Using routinely collected vital signs and laboratory findings, a composite 5-day in-hospital mortality risk score, designated medical admissions risk system (MARS), was developed using an iterative approach involving logistic regression and multivariable fractional polynomials. Results are presented as area under receiver operating characteristics curves (AUROC) as well as Hosmer and Lemeshow goodness-of-fit statistics. RESULTS: A total of 10 712 and 3597 unique patients were admitted to SJH and Nenagh Hospital, respectively. The final score included nine variables [age, heart rate, mean arterial pressure, respiratory rate, temperature, urea, potassium (K), haematocrit and white cell count]. The AUROC for 5-day in-hospital mortality was 0.93 [95% confidence interval (CI) 0.92-0.94] for the SJH cohort (Hosmer and Lemeshow test, P = 0.32) and 0.92 (95% CI 0.90-0.94) for the external Nenagh hospital validation cohort (Hosmer and Lemeshow test, P = 0.28). CONCLUSION: In-hospital mortality estimation using only routinely collected emergency department admission data is possible in unselected acute medical patients using the MARS system. Such a score applied to acute medical patients at the time of admission, could assist senior clinical decision makers in promptly and accurately focusing limited clinical resources. Further studies validating the impact of this model on clinical outcomes are warranted.
INTRODUCTION: Human Immunodeficiency Virus (HIV) has an estimated prevalence of 0.9% in India (5.2 million). Anti-retroviral drugs (ARV) are the treatments of choice and non-adherence is an important factor in treatment failure and development of resistance, as well as being a powerful predictor of survival. This study assesses adherence to ARV in HIV positive patients in Bangalore, India, a country where only 10% of those who need therapy are receiving it. METHODS: A cross-sectional anonymous questionnaire survey of 60 HIV antibody positive patients was carried out with patients attending HIV outpatient services at two centres: The Chest and Maternity Centre, Rajajinagar, and Wockhardt Hospital and Heart Institute, Bangalore. Consent was obtained. Translation was done by a translator and doctors where required. Data was analysed using SPSS statistical analysis. RESULTS: A response rate of 88% (53/60) was achieved. The mean patient age was 39.98 years, with 50% aged 30-40, and 73.6% of participants being male. Mean family size was 4.8 (1-13). 21% lived less than 50 kms and 21% greater than 400 kms from clinic.60% reported they were fully adherent. Adherence was statistically significantly linked to regular follow-up attendance (70.5%, p = 0.002). No other results were statistically significant but trends were found. "100% adherence" trends were seen in older patients, male gender, those from larger families, those who had a previous AIDS defining illness, those taking fewer tablets, and without food restrictions. Commonest side-effects causing non-adherence were metabolic reasons (66%) and GI symptoms (50%). No trends were seen for education level, family income, distance travelled to clinic, time since diagnosis, or time on ART. CONCLUSION: Regular attendance for follow up was statistically significant for 100% lifetime adherence. Positive trends were seen in those in larger families, older, those who had AIDS defining illness, simple regimes, and without side-effects. Education, income, distance travelled and length of time diagnosed or treated had no effect on adherence.
