Abstract The origin of the thermal springs of Bath (England) remains unknown. As part of a programme of research into the structure of the thermal aquifer, the Carboniferous Limestone, an urban reflection seismic survey has been carried out to explore the deep geology of the Bath area. Existing gravity data have been used to provisionally identify the seismic reflectors and to map the depth of the (interpreted) Carboniferous Limestone in the area around the springs. The new seismic data show that at a distance of 2.1 km south-west of the springs, the depth of the (interpreted) Carboniferous Limestone surface increases from 0.4 km below Ordnance Datum (OD) to 1.35 km below OD within a distance of 1.8 km, an average apparent dip of nearly 30 degrees. In all other directions from the springs, the Carboniferous Limestone surface is at a depth of 300 m or less below OD. The work described in this paper is part of a continuing research programme.
BACKGROUND: Diagnosing heart failure and left ventricular systolic dysfunction is difficult on clinical grounds alone. We sought to determine the accuracy of a heart failure register in a single primary care practice, and to examine the usefulness of b-type (or brain) natriuretic peptide (BNP) assay for this purpose. METHODS: A register validation audit in a single general practice in the UK was carried out. Of 217 patients on the heart failure register, 56 of 61 patients who had not been previously investigated underwent 12-lead electrocardiography and echocardiography within the practice site. Plasma was obtained for BNP assay from 45 subjects, and its performance in identifying echocardiographic abnormalities consistent with heart failure was assessed by analysing area under receiver operator characteristic (ROC) curves. RESULTS: 30/217 were found to have no evidence to suggest heart failure on notes review and were probably incorrectly coded. 70/112 who were previously investigated were confirmed to have heart failure. Of those not previously investigated, 24/56 (42.9%) who attended for the study had echocardiographic left ventricular systolic dysfunction. A further 8 (14.3%) had normal systolic function, but had left ventricular hypertrophy or significant valve disease. Overall, echocardiographic features consistent with heart failure were found in only 102/203 (50.2%). BNP was poor at discriminating those with and without systolic dysfunction (area under ROC curve 0.612), and those with and without any significant echocardiographic abnormality (area under ROC curve 0.723). CONCLUSION: In this practice, half of the registered patients did not have significant cardiac dysfunction. On-site echocardiography identifies patients who can be removed from the heart failure register. The use of BNP assay to determine which patients require echocardiography is not supported by these data.
During the early 1920s the Communist Party of Canada (CPC) established a significant presence in industrial Cape Breton, based on support from the militant minority of the district's unusually combative coal mining union, UMWA District 26, and its charismatic Secretary-Treasurer James Bryson McLachlan. The latter's early recruitment was centrally important in forging what would become enduring political links with rank-and-file militants; it was largely thanks to McLachlan that, through all the ebb and flow of social context and human agency, the party survived the vicissitudes of deteriorating structural conditions and its own tactical blunders (most notably during the third period), to emerge in the mid-1930s with genuinely optimistic prospects. The Scot did not always lead his forces well: like many revolutionary contemporaries his bolshevik temperament was ill-suited to defensive struggle. He nevertheless perceived, and to a limited extent acted upon, the need to construct a defensive communist counter-culture within the struggle for workers' power in the community and the workplace. The embodiment of bolshevik intransigence, McLachlan offered a fixed rallying-point for class fighters, especially those who shared his internationalist perspective. Having drawn a new layer of younger militants towards him in 1934-35 — and having seen off the CCF in the process - McLachlan could claim to have placed the CPC in its strongest position for a decade. Tout comme plusieurs revolutionnaires contemporains dont le temperament bolcheviste supportait mal une strategie defensive, le leader ecossais n'a pas toujours su bien mener ses forces. Il a neanmoins reconnu le besoin et parfois meme contribue a l'essor d'une contre-culture communiste defensive dans la lutte pour le pouvoir ouvrier au sein de la communaute et du milieu de travail. Ayant attire dans son orbite une nouvelle generation de militants, tout en abandonnant le part: CCF, McLachlan pouvait se feliciter d'avoir place le PCC, en 1934-35, dans sa plus forte position politique depuis une decennie.
ABSTRACT The Asian tiger mosquito, Aedes albopictus , is currently the most widespread invasive mosquito species in the world. It poses a significant threat to human health, as it is a vector for several arboviruses. We used a SNP chip to genotype 748 Ae. albopictus mosquitoes from 41 localities across Europe, 28 localities in the native range in Asia, and 4 in the Americas. Using multiple algorithms, we examined population genetic structure and differentiation within Europe and across our global dataset to gain insight into the origin of the invasive European populations. We also compared results from our SNP data to those obtained using genotypes from 11 microsatellite loci ( N = 637 mosquitoes from 25 European localities) to explore how sampling effort and the type of genetic marker used may influence conclusions about Ae. albopictus population structure. While some analyses detected more than 20 clusters worldwide, we found mosquitoes could be grouped into 7 distinct genetic clusters, with most European populations originating in East Asia (Japan or China). Interestingly, some populations in Eastern Europe did not share genetic ancestry with any populations from the native range or Americas, indicating that these populations originated from areas not sampled in this study. The SNP and microsatellite datasets found similar patterns of genetic differentiation in Europe, but the microsatellite dataset could not detect the more subtle genetic structure revealed using SNPs. Overall, data from the SNP chip offered a higher resolution for detecting the genetic structure and the potential origins of invasions.
A nurse-led general practice adapted its services to reflect patient feedback and avoid unnecessary referrals to acute care. Changes included planned extended services delivered collaboratively with other practices. The work by Rowley Healthcare to redesign services comes at a time when the NHS is keen to promote and spread innovation in service delivery. In April, health minister Lord Darzi said that strategic health authorities would receive funding to support innovative local projects. An Innovation EXPO where NHS staff can learn about innovations is being held this month in London on 18 and 19 June (NHS Institute for Innovation and Improvement, 2009).
The resignation of J.B. McLachlan from the Communist Party of Canada in 1936 is one of the more controversial episodes in the biography of a well-known Canadian labour radical. He was one of the few party leaders to enjoy wide recognition and popular support. His resignation was a difficult personal decision as well as a significant episode in the history of the party. In previous accounts his resignation has been presented alternately as a repudiation of labour radicalism generally and the Communist Party in particular, as a protest against the adoption of the united front in 1935, or as a form of local and personal political exceptionalism. McLachlan himself made no formal public announcement of his resignation and, except for an impromptu speech at a public meeting in September 1936, he remained largely silent. In response to a letter from party general secretary Tim Buck he prepared a personal explanation of his withdrawal from the party in June 1936. This document, which is reproduced at the end of this article, remains the most important single piece of evidence concerning his resignation. An analysis of the circumstances leading to McLachlan's resignation shows that he did not regard his resignation as a repudiation of basic principles. He had supported the move towards the united front both internationally and domestically but disagreed with the implementation of the policy by the party leadership, especially as demonstrated in the case of the Amalgamated Mine Workers of Nova Scotia. McLachlan's view of the united front, which he considered to be consistent with the position of the Communist International, stressed the principles of internal democracy and local autonomy in the construction of the united front. In McLachlan's view there were already enough indications to show that leaders such as John L. Lewis had not been fundamentally transformed and that in the long run the decision to endorse an alliance of convenience with the established labour bureaucracy was an ambiguous legacy for the class struggle. In 1936 McLachlan was overtaken by events, but given his own history he was in a position to perceive the difficulties ahead more clearly than most of his contemporaries. Resume La demission de J.B.
