Worldwide trends in hypertension prevalence and progress in treatment and control from 1990 to 2019: a pooled analysis of 1201 population-representative studies with 104 million participants

BACKGROUND: Hypertension can be detected at the primary health-care level and low-cost treatments can effectively control hypertension. We aimed to measure the prevalence of hypertension and progress in its detection, treatment, and control from 1990 to 2019 for 200 countries and territories. METHODS: We used data from 1990 to 2019 on people aged 30-79 years from population-representative studies with measurement of blood pressure and data on blood pressure treatment. We defined hypertension as having systolic blood pressure 140 mm Hg or greater, diastolic blood pressure 90 mm Hg or greater, or taking medication for hypertension. We applied a Bayesian hierarchical model to estimate the prevalence of hypertension and the proportion of people with hypertension who had a previous diagnosis (detection), who were taking medication for hypertension (treatment), and whose hypertension was controlled to below 140/90 mm Hg (control). The model allowed for trends over time to be non-linear and to vary by age. FINDINGS: The number of people aged 30-79 years with hypertension doubled from 1990 to 2019, from 331 (95% credible interval 306-359) million women and 317 (292-344) million men in 1990 to 626 (584-668) million women and 652 (604-698) million men in 2019, despite stable global age-standardised prevalence. In 2019, age-standardised hypertension prevalence was lowest in Canada and Peru for both men and women; in Taiwan, South Korea, Japan, and some countries in western Europe including Switzerland, Spain, and the UK for women; and in several low-income and middle-income countries such as Eritrea, Bangladesh, Ethiopia, and Solomon Islands for men. Hypertension prevalence surpassed 50% for women in two countries and men in nine countries, in central and eastern Europe, central Asia, Oceania, and Latin America. Globally, 59% (55-62) of women and 49% (46-52) of men with hypertension reported a previous diagnosis of hypertension in 2019, and 47% (43-51) of women and 38% (35-41) of men were treated. Control rates among people with hypertension in 2019 were 23% (20-27) for women and 18% (16-21) for men. In 2019, treatment and control rates were highest in South Korea, Canada, and Iceland (treatment >70%; control >50%), followed by the USA, Costa Rica, Germany, Portugal, and Taiwan. Treatment rates were less than 25% for women and less than 20% for men in Nepal, Indonesia, and some countries in sub-Saharan Africa and Oceania. Control rates were below 10% for women and men in these countries and for men in some countries in north Africa, central and south Asia, and eastern Europe. Treatment and control rates have improved in most countries since 1990, but we found little change in most countries in sub-Saharan Africa and Oceania. Improvements were largest in high-income countries, central Europe, and some upper-middle-income and recently high-income countries including Costa Rica, Taiwan, Kazakhstan, South Africa, Brazil, Chile, Turkey, and Iran. INTERPRETATION: Improvements in the detection, treatment, and control of hypertension have varied substantially across countries, with some middle-income countries now outperforming most high-income nations. The dual approach of reducing hypertension prevalence through primary prevention and enhancing its treatment and control is achievable not only in high-income countries but also in low-income and middle-income settings. Zika virus infection complicated by Guillain-Barré syndrome – case report, French Polynesia, December 2013

Zika fever, considered as an emerging disease of arboviral origin, because of its expanding geographic area, is known as a benign infection usually presenting as an influenza-like illness with cutaneous rash. So far, Zika virus infection has never led to hospitalisation. We describe the first case of Guillain-Barré syndrome (GBS) occurring immediately after a Zika virus infection, during the current Zika and type 1 and 3 dengue fever co-epidemics in French Polynesia.

To the Editor: Isolated in 1947 from a rhesus monkey in Zika forest, Uganda, Zika virus (ZIKV) is a mosquito-borne flavivirus (1). For half a century, ZIKV was described only as causing sporadic human infections in Africa and Asia, which was mostly confirmed by serologic methods (2). In 2007, the first ZIKV outbreak reported outside Africa and Asia was retrospectively documented from biological samples of patients on Yap Island, Federated States of Micronesia, North Pacific, who had received an incorrect diagnosis of dengue virus (DENV) (3,4). We report here the early investigations that led to identification of ZIKV as the causative agent of an outbreak that started in October 2013 in French Polynesia.

Potential for Zika virus transmission through blood transfusion demonstrated during an outbreak in French Polynesia, November 2013 to February 2014

Since October 2013, French Polynesia has experienced the largest documented outbreak of Zika virus (ZIKAV) infection. To prevent transmission of ZIKAV by blood transfusion, specific nucleic acid testing of blood donors was implemented. From November 2013 to February 2014: 42 (3%) of 1,505 blood donors, although asymptomatic at the time of blood donation, were found positive for ZIKAV by PCR. Our results serve to alert blood safety authorities about the risk of post-transfusion Zika fever.

Zika Virus Infection — After the Pandemic

Zika virus (ZIKV) was discovered in Africa in 1947 and was first detected in Asia in 1966, yet its potential effect on public health was not recognized until the virus caused outbreaks in the Pacific from 2007 to 2015 and began spreading throughout the Americas in 2015.1,2 The ability of ZIKV to cause congenital defects in fetuses and infants, as exemplified by the microcephaly epidemic in Brazil, is an unprecedented feature in a mosquito-borne viral infection. 2-4 Although transmission of ZIKV has declined in the Americas, outbreaks and infection clusters continue to occur in some regions, such as India and Southeast Asia, where there are large populations of women of childbearing age who are susceptible to the virus.5 We review the body of information that was acquired during the pandemic and discuss the epidemiologic trends, current knowledge about the transmission and natural history of ZIKV infection and its sequelae, and the principles of diagnosis and clinical management.

Diffuse reflectance of oceanic shallow waters: Influence of water depth and bottom albedo

We used simplifying assumptions to derive analytical formulae expressing the reflectance of shallow waters as a function of observation depth and of bottom depth and albedo. These formulae also involve two apparent optical properties of the water body: a mean diffuse attenuation coefficient and a hypothetical reflectance which would be observed if the bottom was infinitely deep. The validity of these approximate formulae was tested by comparing their outputs with accurate solutions of the radiative transfer obtained under the same boundary conditions by Monte Carlo simulations. These approximations were also checked by comparing the reflectance spectra for varying bottom depths and compositions determined in coastal lagoons with those predicted by the formulae. These predictions were based on separate determinations of the spectral albedos of typical materials covering the floor, such as coral sand and various green or brown algae. The simple analytical expressions are accurate enough for most practical applications and also allow quantitative discussion of the limitations of remote‐sensing techniques for bottom recognition and bathymetry.

FREQUENCY OF SOLAR-LIKE SYSTEMS AND OF ICE AND GAS GIANTS BEYOND THE SNOW LINE FROM HIGH-MAGNIFICATION MICROLENSING EVENTS IN 2005-2008

We present the first measurement of planet frequency beyond the "snow line" for planet/star mass-ratios[-4.5200) microlensing events during 2005-8. The sample host stars have typical mass M_host 0.5 Msun, and detection is sensitive to planets over a range of projected separations (R_E/s_max,R_E*s_max), where R_E 3.5 AU sqrt(M_host/Msun) is the Einstein radius and s_max (q/5e-5)^{2/3}, corresponding to deprojected separations ~3 times the "snow line". Though frenetic, the observations constitute a "controlled experiment", which permits measurement of absolute planet frequency. High-mag events are rare, but the high-mag channel is efficient: half of high-mag events were successfully monitored and half of these yielded planet detections. The planet frequency derived from microlensing is a factor 7 larger than from RV studies at factor ~25 smaller separations [2

Different secretory repertoires control the biomineralization processes of prism and nacre deposition of the pearl oyster shell

Mollusca evolutionary success can be attributed partly to their efficiency to sustain and protect their soft body with an external biomineralized structure, the shell. Current knowledge of the protein set responsible for the formation of the shell microstructural polymorphism and unique properties remains largely patchy. In Pinctada margaritifera and Pinctada maxima, we identified 80 shell matrix proteins, among which 66 are entirely unique. This is the only description of the whole "biomineralization toolkit" of the matrices that, at least in part, is thought to regulate the formation of the prismatic and nacreous shell layers in the pearl oysters. We unambiguously demonstrate that prisms and nacre are assembled from very different protein repertoires. This suggests that these layers do not derive from each other.

Scientists’ warning – The outstanding biodiversity of islands is in peril

Despite islands contributing only 6.7% of land surface area, they harbor ~20% of the Earth's biodiversity, but unfortunately also ~50% of the threatened species and 75% of the known extinctions since the European expansion around the globe. Due to their geological and geographic history and characteristics, islands act simultaneously as cradles of evolutionary diversity and museums of formerly widespread lineages-elements that permit islands to achieve an outstanding endemicity. Nevertheless, the majority of these endemic species are inherently vulnerable due to genetic and demographic factors linked with the way islands are colonized. Here, we stress the great variation of islands in their physical geography (area, isolation, altitude, latitude) and history (age, human colonization, human density). We provide examples of some of the most species rich and iconic insular radiations. Next, we analyze the natural vulnerability of the insular biota, linked to genetic and demographic factors as a result of founder events as well as the typically small population sizes of many island species. We note that, whereas evolution toward island syndromes (including size shifts, derived insular woodiness, altered dispersal ability, loss of defense traits, reduction in clutch size) might have improved the ability of species to thrive under natural conditions on islands, it has simultaneously made island biota disproportionately vulnerable to anthropogenic pressures such as habitat loss, overexploitation, invasive species, and climate change. This has led to the documented extinction of at least 800 insular species in the past 500 years, in addition to the many that had already gone extinct following the arrival of first human colonists on islands in prehistoric times. Finally, we summarize current scientific knowledge on the ongoing biodiversity loss on islands worldwide and express our serious concern that the current trajectory will continue to decimate the unique and irreplaceable natural heritage of the world's islands. We conclude that drastic actions are urgently needed to bend the curve of the alarming rates of island biodiversity loss.

Catecholaminergic polymorphic ventricular tachycardia: RYR2 mutations, bradycardia, and follow up of the patients

BACKGROUND: The aim of the study was to assess underlying genetic cause(s), clinical features, and response to therapy in catecholaminergic polymorphic ventricular tachycardia (CPVT) probands. METHODS AND RESULTS: We identified 13 missense mutations in the cardiac ryanodine receptor (RYR2) in 12 probands with CPVT. Twelve were new, of which two are de novo mutations. A further 11 patients were silent gene carriers, suggesting that some mutations are associated with low penetrance. A marked resting sinus bradycardia off drugs was observed in all carriers. On beta blocker treatment, 98% of the RYR2 mutation carriers remained symptom free with a median follow up of 2 (range: 2-37) years. CONCLUSION: CPVT patients with RYR2 mutation have bradycardia regardless of the site of the mutation, which could direct molecular diagnosis in (young) patients without structural heart disease presenting with syncopal events and a slow heart rate but with normal QTc at resting ECG. Treatment with beta blockers has been very effective in our CPVT patients during initial or short term follow up. Given the risk of sudden death and the efficacy of beta blocker therapy, the identification of large numbers of RYR2 mutations thus calls for genetic screening, early diagnosis, and subsequent preventive strategies.

Invasive alien species on islands: impacts, distribution, interactions and management

SUMMARY Invasive alien species (IASs) on islands have broad impacts across biodiversity, agriculture, economy, health and culture, which tend to be stronger than on continents. Across small-island developing states (SIDSs), although only a small number of IASs are widely distributed, many more, including those with greatest impact, are found on only a small number of islands. Patterns of island invasion are not consistent across SIDS geographic regions, with differences attributable to correlated patterns in island biogeography and human development. We identify 15 of the most globally prevalent IASs on islands. IAS impacts on islands are exacerbated through interactions with a number of other global change threats, including over-exploitation, agricultural intensification, urban development and climate change. Biosecurity is critical in preventing IAS invasion of islands. Eradication of IASs on islands is possible at early stages of invasion, but otherwise is largely restricted to invasive mammals, or otherwise control is the only option. Future directions in IAS management and research on islands must consider IASs within a broader portfolio of threats to species, ecosystems and people's livelihoods on islands. We advocate for stronger collaborations among island countries and territories faced with the same IASs in similar socio-ecological environments.

Parallel Evolution of Nacre Building Gene Sets in Molluscs

The capacity to biomineralize is closely linked to the rapid expansion of animal life during the early Cambrian, with many skeletonized phyla first appearing in the fossil record at this time. The appearance of disparate molluscan forms during this period leaves open the possibility that shells evolved independently and in parallel in at least some groups. To test this proposition and gain insight into the evolution of structural genes that contribute to shell fabrication, we compared genes expressed in nacre (mother-of-pearl) forming cells in the mantle of the bivalve Pinctada maxima and the gastropod Haliotis asinina. Despite both species having highly lustrous nacre, we find extensive differences in these expressed gene sets. Following the removal of housekeeping genes, less than 10% of all gene clusters are shared between these molluscs, with some being conserved biomineralization genes that are also found in deuterostomes. These differences extend to secreted proteins that may localize to the organic shell matrix, with less than 15% of this secretome being shared. Despite these differences, H. asinina and P. maxima both secrete proteins with repetitive low-complexity domains (RLCDs). Pinctada maxima RLCD proteins-for example, the shematrins-are predominated by silk/fibroin-like domains, which are absent from the H. asinina data set. Comparisons of shematrin genes across three species of Pinctada indicate that this gene family has undergone extensive divergent evolution within pearl oysters. We also detect fundamental bivalve-gastropod differences in extracellular matrix proteins involved in mollusc-shell formation. Pinctada maxima expresses a chitin synthase at high levels and several chitin deacetylation genes, whereas only one protein involved in chitin interactions is present in the H. asinina data set, suggesting that the organic matrix on which calcification proceeds differs fundamentally between these species. Large-scale differences in genes expressed in nacre-forming cells of Pinctada and Haliotis are compatible with the hypothesis that gastropod and bivalve nacre is the result of convergent evolution. The expression of novel biomineralizing RLCD proteins in each of these two molluscs and, interestingly, sea urchins suggests that the evolution of such structural proteins has occurred independently multiple times in the Metazoa.

Support Vector Machine for Multifrequency SAR Polarimetric Data Classification

The objective of this paper is twofold: first, to assess the potential of radar data for tropical vegetation cartography and, second, to evaluate the contribution of different polarimetric indicators that can be derived from a fully polarimetric data set. Because of its ability to take numerous and heterogeneous parameters into account, such as the various polarimetric indicators under consideration, a support vector machine (SVM) algorithm is used in the classification step. The contribution of the different polarimetric indicators is estimated through a greedy forward and backward method. Results have been assessed with AIRSAR polarimetric data polarimetric data acquired over a dense tropical environment. The results are compared to those obtained with the standard Wishart approach, for single frequency and multifrequency bands. It is shown that, when radar data do not satisfy the Wishart distribution, the SVM algorithm performs much better than the Wishart approach, when applied to an optimized set of polarimetric indicators.

An Alternative to Static Climatologies: Robust Estimation of Open Ocean CO2 Variables and Nutrient Concentrations From T, S, and O2 Data Using Bayesian Neural Networks

This work presents two new methods to estimate oceanic alkalinity (AT), dissolved inorganic carbon (CT), pH, and pCO2 from temperature, salinity, oxygen, and geolocation data. “CANYON-B” is a Bayesian neural network mapping that accurately reproduces GLODAPv2 bottle data and the biogeochemical relations contained therein. “CONTENT” combines and refines the four carbonate system variables to be consistent with carbonate chemistry. Both methods come with a robust uncertainty estimate that incorporates information from the local conditions. They are validated against independent GO-SHIP bottle and sensor data, and compare favourably to other state-of-the-art mapping methods. As "dynamic climatologies" they show comparable performance to classical climatologies on large scales but a much better representation on smaller scales (40–120 d, 500–1500 km) compared to in situ data. The limits of these mappings are explored with pCO2 estimation in surface waters, i.e., at the edge of the domain with high intrinsic variability. In highly productive areas, there is a tendency for pCO2 overestimation due to decoupling of the O2 and C cycles by air-sea gas exchange, but global surface pCO2 estimates are unbiased compared to a monthly climatology. CANYON-B and CONTENT are highly useful as transfer functions between components of the ocean observing system (GO-SHIP repeat hydrography, BGC-Argo, underway observations) and permit the synergistic use of these highly complementary systems, both in spatial/temporal coverage and number of observations. Through easily and robotically-accessible observations they allow densification of more difficult-to-observe variables (e.g., 15 times denser AT and CT compared to direct measurements). At the same time, they give access to the complete carbonate system. This potential is demonstrated by an observation-based global analysis of the Revelle buffer factor, which shows a significant, high latitude-intensified increase between +0.1 and +0.4 units per decade. This shows the utility that such transfer functions with realistic uncertainty estimates provide to ocean biogeochemistry and global climate change research. In addition, CANYON-B provides robust and accurate estimates of nitrate, phosphate, and silicate.

Effects of Virgin Micro- and Nanoplastics on Fish: Trends, Meta-Analysis, and Perspectives

Environmental plastic pollution is a major ecological and societal concern today. Over the past decade, a broad range of laboratory and experimental studies have complemented field observations in the hope of achieving a better understanding of the fate and impact of micro- and/or nanoplastics (MP/NP) on diverse organisms (e.g., birds, fish, and mammals). However, plastic pollution remains challenging to monitor in the environment and to control under laboratory conditions, and plastic particles are often naturally or experimentally co-contaminated with diverse chemical pollutants. Therefore, our understanding of the effects of virgin MP/NP in freshwater and marine fish is still limited. Here, we performed a systematic review of the most up-to-date literature on the effects of virgin MP/NP on fish under laboratory conditions. A total of 782 biological endpoints investigated in 46 studies were extracted. Among these endpoints, 32% were significantly affected by exposure to virgin MP/NP. More effects were observed for smaller plastic particles (i.e., size ≤20 μm), affecting fish behavioral and neurological functions, intestinal permeability, metabolism, and intestinal microbiome diversity. In addition, we propose suggestions for new research directions to lead toward innovative, robust, and scientifically sound experiments in this field. This review of experimental studies reveals that the toxicity of virgin MP/NP on fish should be more systematically evaluated using rigorous laboratory-based methods and aims toward a better understanding of the underlying mechanisms of this toxicity to fish.

Change in mean temperature as a predictor of extreme temperature change in the Asia–Pacific region

Abstract Trends (1961–2003) in daily maximum and minimum temperatures, extremes and variance were found to be spatially coherent across the Asia–Pacific region. The majority of stations exhibited significant trends: increases in mean maximum and mean minimum temperature, decreases in cold nights and cool days, and increases in warm nights. No station showed a significant increase in cold days or cold nights, but a few sites showed significant decreases in hot days and warm nights. Significant decreases were observed in both maximum and minimum temperature standard deviation in China, Korea and some stations in Japan (probably reflecting urbanization effects), but also for some Thailand and coastal Australian sites. The South Pacific convergence zone (SPCZ) region between Fiji and the Solomon Islands showed a significant increase in maximum temperature variability. Correlations between mean temperature and the frequency of extreme temperatures were strongest in the tropical Pacific Ocean from French Polynesia to Papua New Guinea, Malaysia, the Philippines, Thailand and southern Japan. Correlations were weaker at continental or higher latitude locations, which may partly reflect urbanization. For non‐urban stations, the dominant distribution change for both maximum and minimum temperature involved a change in the mean, impacting on one or both extremes, with no change in standard deviation. This occurred from French Polynesia to Papua New Guinea (except for maximum temperature changes near the SPCZ), in Malaysia, the Philippines, and several outlying Japanese islands. For urbanized stations the dominant change was a change in the mean and variance, impacting on one or both extremes. This result was particularly evident for minimum temperature. The results presented here, for non‐urban tropical and maritime locations in the Asia–Pacific region, support the hypothesis that changes in mean temperature may be used to predict changes in extreme temperatures. At urbanized or higher latitude locations, changes in variance should be incorporated. Copyright © 2005 Royal Meteorological Society.

Transcriptome and proteome analysis of Pinctada margaritifera calcifying mantle and shell: focus on biomineralization

BACKGROUND: The shell of the pearl-producing bivalve Pinctada margaritifera is composed of an organic cell-free matrix that plays a key role in the dynamic process of biologically-controlled biomineralization. In order to increase genomic resources and identify shell matrix proteins implicated in biomineralization in P. margaritifera, high-throughput Expressed Sequence Tag (EST) pyrosequencing was undertaken on the calcifying mantle, combined with a proteomic analysis of the shell. RESULTS: We report the functional analysis of 276 738 sequences, leading to the constitution of an unprecedented catalog of 82 P. margaritifera biomineralization-related mantle protein sequences. Components of the current "chitin-silk fibroin gel-acidic macromolecule" model of biomineralization processes were found, in particular a homolog of a biomineralization protein (Pif-177) recently discovered in P. fucata. Among these sequences, we could show the localization of two other biomineralization protein transcripts, pmarg-aspein and pmarg-pearlin, in two distinct areas of the outer mantle epithelium, suggesting their implication in calcite and aragonite formation. Finally, by combining the EST approach with a proteomic mass spectrometry analysis of proteins isolated from the P. margaritifera shell organic matrix, we demonstrated the presence of 30 sequences containing almost all of the shell proteins that have been previously described from shell matrix protein analyses of the Pinctada genus. The integration of these two methods allowed the global composition of biomineralizing tissue and calcified structures to be examined in tandem for the first time. CONCLUSIONS: This EST study made on the calcifying tissue of P. margaritifera is the first description of pyrosequencing on a pearl-producing bivalve species. Our results provide direct evidence that our EST data set covers most of the diversity of the matrix protein of P. margaritifera shell, but also that the mantle transcripts encode proteins present in P. margaritifera shell, hence demonstrating their implication in shell formation. Combining transcriptomic and proteomic approaches is therefore a powerful way to identify proteins involved in biomineralization. Data generated in this study supply the most comprehensive list of biomineralization-related sequences presently available among protostomian species, and represent a major breakthrough in the field of molluskan biomineralization.

Microplastics Affect Energy Balance and Gametogenesis in the Pearl Oyster <i>Pinctada margaritifera</i>

Plastic pollution in the environment is increasing at global scale. Microplastics (MP) are derived from degradation of larger plastic items or directly produced in microparticles form (< 5 mm). Plastics, widely used in structures and equipment of pearl farming, are a source of pollution to the detriment of the lagoon ecosystem. To evaluate the impact of MP on the physiology of Pinctada margaritifera, a species of ecological and commercial interests, adult oysters were exposed to polystyrene microbeads (micro-PS of 6 and 10 μm) for 2 months. Three concentrations, 0.25, 2.5, and 25 μg L–1, and a control were tested. Ingestion and respiration rate and assimilation efficiency were monitored on a metabolic measurement system to determine the individual energy balance (Scope For Growth, SFG). Effects on reproduction were also assessed. The assimilation efficiency decreased significantly according to micro-PS concentration. The SFG was significantly impacted by a dose-dependent decrease from 0.25 μg L–1 (p < 0.0001), and a negative SFG was measured in oysters exposed to 25 μg L–1. Gonads may have provided the missing energy to maintain animals’ metabolism through the production of metabolites derived from germ cells phagocytosis. This study shows that micro-PS significantly impact the assimilation efficiency and more broadly the energy balance of P. margaritifera, with negative repercussions on reproduction.

We present the discovery of a Neptune-mass planet OGLE-2007-BLG-368Lb with a planet-star mass ratio of q = [9.5 2.1] 10 -5 via gravitational microlensing.
